Background and objectives: Skin graft and various flaps are used to cover defects; available options have different patterns of blood supply, aesthetic outcome, donor area morbidity, surrounding soft tissue availability, operative time duration, the need for microsurgical expertise and long term complications. The aim of this study is to assess reliability and versatility of keystone flap for the reconstruction of the skin defects. Methods: Reconstruction of skin defects by keystone flap were done in plastic surgery department between (January 2018- May 2020), records of patient’s age, and cause of the defect, size, flap outcome and complications are documented.to study the ability of the flap to cover the skin defect and its long term survival. Results: Nineteen patients underwent reconstruction, age was ranging from two days to seventy-five-year-old with mean age 37.5 years, twelve were males and seven were females. Size of the defect ranged from (0.5 cm x 1 cm) to (9 cm x 7 cm) with mean defect size 31.75 cm2. Seven flaps in head and neck, five in the trunk, five in lower limb and two flaps in upper limb. All flaps were successful in covering the defects, no partial or total necrosis with minor complication in two patients. Conclusions: Keystone flap is a reliable flap with excellent survival rate, versatile and can be used for skin defects in different regions of the body with low complication rate, easy technique for learning by surgeons and have short operative duration with good cosmetic outcome.

Objectives Somatic symptom disorder (SSD) is a diagnostic classification to describe the occurrence of physical symptoms without organic cause. This study aimed to identify prevalence rates and associated characteristics of SSD among adults in primary care settings. Methods This cross-sectional study took place between July 2020 and March 2021 and included a random selection of 3383 Omani adults attending 12 primary healthcare centres in Muscat Governorate. Screening was conducted using an Arabic version of the Somatic Symptom Scale-8 (SSS-8). Results A total of 2000 adults participated in the study (response rate: 67.3%), of which most were female (71.7%) and under 50 years old (86.2%). Based on their SSS-8 scores, 602 participants (17.8%) had SSD, resulting in an overall prevalence estimate of 30.1% (95% confidence interval: 28.13–32.15). Significant associations were observed between SSD and age ( p = 0.002), gender ( p < 0.001), marital status ( p = 0.030) and chronic comorbidities ( p = 0.001). In addition, adjusted odds ratio estimates revealed SSD to be significantly associated with gender ( p < 0.001), education level ( p < 0.001) and chronic comorbidities ( p = 0.001). Conclusion The estimated prevalence of SSD in primary care settings is high compared to reports from elsewhere in the Gulf region. There is an urgent need to enhance the diagnosis of SSD at the primary care level in order to reduce healthcare service overutilisation and patient dissatisfaction. Moreover, healthcare practitioners should be aware of the effect of age, gender, educational status and chronic comorbidities on somatic symptoms.

Posttraumatic Stress Disorder and Persistent Somatic Symptoms During the COVID-19 Pandemic: The Role of Sense of Threat

Objective: Persistent somatic symptoms, such as pain and fatigue, have been referred to as somatization. Somatization is commonly associated with histories of trauma and posttraumatic stress disorder (PTSD). Although previous research has demonstrated that PTSD can predict somatic problems, there has been no examination of this at the level of PTSD symptom clusters and multidimensional assessment of somatic symptoms. We examined the association between the three International Classification of Disease (11th Edition) PTSD symptom clusters (reexperiencing in the here and now, avoidance, and sense of threat), measured in relation to the COVID-19 pandemic as the stressor, and somatic symptoms while statistically adjusting for confounding variables. Methods: Participants were a nationally representative sample of 1041 adults from the general population of the Republic of Ireland. Physical health problems across the domains of pain, gastrointestinal, cardiopulmonary, and fatigue were assessed by the Patient Health Questionnaire, and PTSD symptoms were assessed using the International Trauma Questionnaire. Results: Sense of threat was associated with the presence of pain (β = 0.254), fatigue (β = 0.332), gastrointestinal (β = 0.234), and cardiovascular symptoms (β = 0.239). Avoidance was associated with pain (β = 0.347). Reexperiencing was not associated with any physical health variable. Conclusions: In the context of COVID-19, the sense of threat symptoms in PTSD is most strongly related to somatic problems. Findings suggest that interventions addressing sense of threat symptoms might provide relief from somatization.
