purpose. To report outcomes of 21 total wrist arthroplasties (TWA) using the Universal 2 prosthesis. Methods. Five men and 14 women aged 44 to 82 (mean, 62) years underwent 21 total wrist arthroplasties for rheumatoid arthritis (n=19) and post-traumatic arthritis (n=2) by a single surgeon using the Universal 2 prosthesis. Pre-and post-operative pain and function were assessed by a single surgeon using the Disabilities of the Arm, Shoulder and Hand (DASH) score and the patient-rated wrist evaluation (PRWE) score. Range of motion, stability, dislocation rate, and neurovascular status were also assessed. Radiographs were evaluated for implant alignment and fit, screw positioning, and implant loosening. results. The mean time to assessment of the range of motion was 3.1 (range, 1.8-3.9) years, and the mean time to assessment of the PRWE score was 4.8 (range, 2.1-7.3) years. The range of motion in each direction and the mean DASH and PRWE scores improved significantly following TWA. Two patients Total wrist arthroplasty using the Universal 2 prosthesis 2012;20(3):365-8 had restricted range of motion, which was treated by manipulation under anaesthetic (after 6 months in one and 8 weeks in the other). One patient underwent excision of a palmar bony bridge. One patient endured extensor pollicis longus rupture and underwent tendon transfer after 5 months. Radiographs revealed no evidence of implant loosening, migration, or malalignment. There was no sign of osteonecrosis in the remaining carpals or metacarpals. conclusion. The Universal 2 TWA achieved significant improvement in range of motion and functional outcome of the wrist, with reduced rates of early joint instability, dislocation, and implant loosening, compared to previous implants. ZusammenfassungDer Rheumapatient, mit seiner Tendenz zur multiplen Gelenkzerstörung und den fehlenden Kompensationsmöglichkeiten der Nachbargelenke, profitiert entscheidend von einer mobilen Handgelenksrekonstruktion mit Erhalt der feinmotorischen Gebrauchsfähigkeit der Hand. Wir berichten über unsere Erfahrungen mit einer zementfrei implantierten Handgelenksprothese, die sich seit 1993 in der klinischen Anwendung befindet. Die Modulare Physiologische Handgelenksprothese repräsentiert eine neue Generation von Handgelenksimplantaten, mit der eine anatomisch-funktionelle Rekonstruktion des Handgelenks mit Wiederaufbau der karpalen Höhe und des Gelenkdrehpunkts mög-lich ist. Material und Methodik: Im Zeitraum 1993 bis Ende 2000 wurden 51 zementfreie Handgelenksprothesen implantiert. 36 Handgelenke wurden mit dem Erstgenerationsmodell (APH) und 15 Patienten mit dem Nachfolgemodell (MPH) versorgt. Bei allen Patienten war die Diagnose einer chronischen Polyarthritis gesichert. Die durchschnittliche Erkrankungsdauer der rheumatoiden Arthritis lag bei 14 Jahren mit einem symptomatischen Handgelenksbefall von 9,7 Jahren. Alle Patienten konnten im Rahmen einer prospektiven Studie klinisch und radiologisch kontrolliert werden. Zwei Patienten sind bereits verstorben. Der Nachuntersuchungszeitraum liegt zwischen zwei und neun Jahren mit einem Mittelwert von 4,2 Jahren. Die Nachuntersuchung basierte auf dem Bewertungsschema nach Clayton. Ergebnisse: Im Bewertungsschema nach Clayton erreichten 72,5 % der operierten Handgelenke ein gutes und sehr gutes Ergebnis, was einer durchschnittlichen Steigerung von präoperativ 46,2 auf postoperativ 76,3 Punkte entspricht. Rheumatoid arthritis often causes symptoms in the wrist; pain and deformity affect the function of the entire hand and predispose deformity of the fingers.Preventive surgery, ie, synovectomy, joint stabilization, and resection of the ulnar head may decrease pain, correct deformity, and avoid tendon ruptures in the hand.If rheumatic disease has destroyed the wrist, total or partial arthrodesis (fusion) is an appropriate way to achieve stabilization and pain relief.Currently, there is no wrist endoprosthesis available which offers practical and reliable long-term results.The data in this chapter are based mainly on retrospective clinical and radiological studies in rheumatic patients.
