Abstract:Retroperitoneoscopy allows safe access to caliceal diverticula regardless of location, and permits complete removal of stone and fulguration of the diverticular neck. Gelatin resorcinol formaldehyde glue minimizes the risk of urinoma formation, and provides a simple, quick and safe alternative for nephrotomy closure.
“…Many authors stress its indication in the case of two contextual diseases, as happens in anatomical variations of location and shape of the kidney, 21-27 pyeloureteral junction dysplasia, 28-32 and collecting system diverticula. [33][34][35][36] The experience with laparoscopy treatment of kidney stones and concomitant peripelvic cysts has not been described before. In our experience peripelvic cysts are an acquired anomaly that could contribute to stone formation and make it difficult to perform endourologic procedures, such as URS, by reducing the diameter of pelvis, limiting passage of instruments and limiting the freedom of flexible tools.…”

Laparoscopic management of urolithiasis should be an alternative choice of treatment in some particular and selected indication. Concomitant symptomatic peripelvic cysts and kidney stones could justify simultaneous laparoscopic management. The procedure is technically challenging but safe and effective, and it avoids a staged treatment.

“…Use of perinephric fat or gelatin resorcinol formaldehyde glue to fill the cavity and seal the diverticular neck has been described. 11 Miller and associates 8 have described the suture repair of the neck of a caliceal diverticulum.…”

“…As we have shown, laparoscopic suture ligation of the diverticular neck can effectively mimic the open technique of surgical closure of the neck, which is the most effective method with a Ͼ95% success rate. 1,[9][10][11] We feel that the anterior diverticulum is best managed by a laparoscopic approach.…”
Patients with a large, symptomatic Type 2 caliceal diverticulum with thin overlying parenchyma are ideal candidates for laparoscopic intervention. The laparoscopic technique of caliceal diverticulum ablation in the pediatric population is feasible in appropriate patients.
“…Laparoscopic retroperitoneal and transperitoneal approaches were proposed and consisted of nephrotomy with treatment of stones and fulguration of the diverticula [2,10,11]. Nevertheless, a thick layer of kidney parenchyma overlying the diverticulum increases the difficulty of laparoscopic nephrotomy, because of bleeding.…”
