Symptoms associated with Lewy body disease in mild cognitive impairment

Abstract: Drooling, misjudging objects and symptoms related to parkinsonism, fluctuating cognition and RBD may be the most characteristic symptoms of MCI-LB. Slowness, tremor, autonomic symptoms and hyposmia are all common in MCI-LB but are not specific to the disease. Copyright © 2017 John Wiley & Sons, Ltd.

“…Symptoms in MCI-LB such as depression, anxiety and delusions were more likely to be reported in this context than when enquired about directly in a symptom questionnaire with both the subject and informant present, as previously reported in this cohort (Donaghy et al, 2017). Conversely, the informant was not aware of visual hallucinations in two out of twelve volunteers that reported visual hallucinations, as the patient had not informed them of their experiences.…”

“…It is now well known that clinical features such as psychiatric symptoms, RBD, and autonomic nervous symptoms characterize the prodromal stage of DLB before cognitive impairment appears. 5,6,9,14 However, initial symptoms and prodromal symptoms have not yet been fully clarified. Although reports regarding the incidence of DLBrelated symptoms at the time of diagnosis have been published, [15][16][17][18] to our knowledge, there have been no previous detailed investigations of initial symptoms.…”

“…[8][9][10] Participants with DLB generally perform better on episodic memory tests than AD participants for any given level of dementia severity and are more likely to improve with cued recall and recognition. [8][9][10] Participants with DLB generally show milder naming deficits than participants with AD on measures of confrontation naming, while DLB participants may perform worse than AD in category and letter fluency tasks, 4 due in part to difficulties with verbal initiation in timed tasks and attentional deficits. Hallucinations and delusions are common in DLB, elicited primarily through informant interviews and less so from participant reports or direct observation by clinicians.…”
