Whenever we are confronted with action opportunities in everyday life, e.g., when passing an opening, we rely on our ability to precisely estimate our own bodily capabilities in relation to the environmental conditions. So-called affordance judgments can be affected after brain damage. Previous studies with healthy adults showed that such judgments appeared to be trainable within one session. In the current study, we examined whether stroke patients with either right brain damage (n = 30) or left brain damage (n = 30) may similarly profit from training in an aperture task. Further, the role of neuropsychological deficits in trainability was investigated. In the administered task, stroke patients decided whether their hand would fit into a presented opening with varying horizontal width (Aperture Task). During one training session, patients were asked to try to fit their hand into the opening and received feedback on their decisions. We analyzed accuracy and the detection theory parameters perceptual sensitivity and judgment tendency. Both patients with right brain damage and patients with left brain damage showed improved performance during training as well as post training. High variability with differential profiles of trainability was revealed in these patients. Patients with impaired performance in a visuo-spatial or motor-cognitive task appeared to profit considerably from the target-driven action phase with feedback, but the performance increase in judgments did not last when the action was withdrawn. Future studies applying lesion analysis with a larger sample may shed further light on the dissociation in the trainability of affordance judgments observed in patients with versus without visuo-spatial or motor-cognitive deficits.

Introduction: For limb apraxia ‒ a heterogeneous disorder of higher motor cognition following stroke ‒ an enduring debate has arisen regarding the existence of dissociating neural correlates for finger and hand gestures in the left hemisphere. We re-assessed this question asking whether previous attempts analysing pooled samples of patients with deficits in only one and patients with deficits in both imitation types might have led to systematically biased results. Methods: We conducted frequentist and Bayesian, voxelwise and regionwise lesion symptom mappings on a pooled sample (N=96) and subsamples containing only shared and only isolated hand and finger imitation deficits and respective controls. Results: Anatomical analyses on the isolated sample reinforced a cortical dissociation of finger deficits (located more anteriorly) and hand deficits (located more posteriorly). The presence of patients with shared deficits did indeed dilute associations that appeared stronger in the respective isolated samples. Also, brain regions truly associated with hand imitation deficits showed a positive bias for finger imitation deficits, when the sample contained patients with shared deficits. In addition, our frequentist parameters uncovered that some of our Bayesian evidence supported reverse associations (damage protecting from rather than increasing the deficit). Discussion: Joint analyses of patients with shared and isolated imitation deficits do indeed lead to biases, which may explain why some previous studies have failed to detect the actual neural dissociation between hand and finger imitation deficits.

Limb apraxia is an acquired higher-order motor disorder affecting skilful actions, often observed following a stroke. It is assessed on tasks involving gesture production, recognition, imitation, object use, and multi-object sequencing. A two-system model for the organisation of action has been hypothesised based on errors patients make on these tasks. This distinguishes between deficits conceptualising actions mediated by an ‘indirect’ route to action, and deficits in implementing skilful actions, mediated via a ‘direct’ route. We tested 29 stroke patients with left hemisphere lesions on comprehensive apraxic tasks provided by the Birmingham Cognitive Screening. We employed white matter disconnection mapping, derived from patients’ structural T1 lesions and a diffusion-weighted dataset composed of 176 healthy participants, acquired with 7 T MRI scans from the human connectome project (HCP). Our analysis revealed that deficits in meaningless gesture imitation were associated with significant disconnection of the left and right occipital lobes via the splenium of the corpus callosum. This finding highlights a possible role of the right hemisphere in mediating ‘direct’ pathway- visuomotor deficits associated with apraxia. Though we did not identify a significant difference in tasks pertaining to an ‘indirect’ pathway in apraxia, there was a trend toward disconnection of the left fornix in gesture production tasks. Our results suggest separable disconnections associated with perceptual ‘direct’ and, possibly, more conceptual (memory-related) ‘indirect’ pathways. Further research with larger patient cohorts is needed to confirm and expand upon these findings, enhancing our understanding of the intricate neurobehavioural consequences of stroke, such as limb apraxia.
