Cancer has become one of the leading causes of morbidity and mortality worldwide. This disease is classified broadly by tissue, organ, and system; different cancer types and subtypes require different treatments. Drug bioavailability, selectivity, and high dosage, as well as extended treatment, are significantly associated with the development of resistance – a complex problem in cancer therapy. It is expected that the combination of anticancer drugs and drug delivery systems, using polymers to increase the access of such agents to their site of action, will improve the efficacy of therapy. Polyethyleneimine (PEI) is a polymer used as a co-delivery system for anticancer drugs and gene therapy. PEI is also useful for other purposes, such as transfection and bio-adsorbent agents. In co-delivery, PEI can promote drug internalization. However, PEI with a high molecular weight is linked to higher cytotoxicity, thus requiring further evaluation of clinical safety. This review focuses on the utilization of PEI as a co-delivery system for anticancer therapy, as well as its potential to overcome resistance, particularly in the treatment of specific subtypes (eg, breast cancer). In conclusion, PEI has promising applications and is improvable for the development of anticancer drugs.

In recent decades, polymeric materials have been extensively investigated for their widespread therapeutic applications in various biomedical fields. In particular, biomolecule conjugated polymers continue to draw attention due to their distinctive physicochemical and biological properties. By appending biomolecules on polymeric constructs, these synthetic macromolecules can be strategized as multimotive theranostic agents. In this Review, we intend to spotlight the biomedical applications of polymeric materials containing cholic acid (CA) as the biomolecular counterpart. CA is the most abundantly produced bile acid during cholesterol metabolism in the human body and is also the least expensive one. The fascinating properties such as inimitable facial amphiphilicity, convenient bioavailability, trivial cytotoxicity, and excellent biocompatibility make CA unique to design numerous cholic acid-based polymers (CAPs) in the diverse areas of biomedical research. The primary focus of this Review is to highlight the recent achievements (2011−present) on the obligatory role of the CA moiety in CAPs in these biomedical applications. Especially the therapeutic applications which include drug delivery, gene transfection, antimicrobial activity, bioimaging and diagnosis, wound healing, and other miscellaneous bioapplications are highlighted in this Review.

Cholic acid-conjugated polyethylenimine 25K (PEI 25K-CA) was synthesized and characterized for gene delivery of its buffer capacity, PEI 25K-CA/plasmid DNA complex, zeta potential and particle size, transfection efficiency, cytotoxicity, and carcinoma cell growth inhibition. PEI 25K-CA was complexed with plasmid DNA at the ratio above 0.25/1. PEI 25K-CA/p53-EGFP (p53-enhanced green fluorescent protein) and PEI 25K/p53-EGFP had the particle sizes of 238.2 ± 3.1-529.1 ± 3.6 nm, and the zeta potential of 3.7 ± 1.6-4.3 ± 0.2 mV, respectively. The particle size of the complexes increased with the period of time. PEI 25K-CA/p53-EGFP had transfection efficiency as PEI 25K/ p53-EGFP. The cytotoxicity of PEI 25K-CA was dependent on the concentration of the polymer. The growth inhibition activity was dependent on the dose of p53-EGFP. PEI 25K-CA/pEGFP had better colloidal stability than PEI 25K-CA/ pEGFP. This study concluded that PEI 25K-CA had efficient gene delivery activity as PEI and had lower cytotoxicity and more colloidal stability than PEI 25K.
