Synthetic Porcine Secretin Is Highly Accurate in Pancreatic Function Testing in Individuals with Chronic Pancreatitis

Abstract: The secretin stimulation test is the most sensitive and specific test for pancreatic function. It is usually performed using biologically derived porcine secretin. Several shortages of biologic porcine secretin have occurred in the past few years. The aim of this study was to compare synthetic porcine secretin to biologic porcine secretin in pancreatic function testing in subjects with chronic pancreatitis. Twelve patients with a previously abnormal secretin stimulation test were enrolled. After an overnight f… Show more

“…Synthetic secretin was administered in a dose of 0.2 g/kg intravenously, up to a maximum of 16 g, over a 1-minute period. Synthetic secretin has been compared favorably to the natural porcine secretin in the diagnosis of pancreatic exocrine insufficiency (26). As we have subjectively observed that pancreatic ductal response is similar with the two forms of secretin, patients who underwent MRP with either porcinederived or synthetic secretin were included in the study.…”

“…Biological porcine secretin was in widespread use until a shortage developed a few years ago. Synthetic porcine secretin 40 and recombinant human secretin 41 subsequently became available for research purposes, and comparison studies between all three secretin preparations revealed no statistically significant differences in the accuracy for the diagnosis of chronic pancreatitis 40 ,. 41 A synthetic porcine secretin preparation has now been approved by the Food and Drug Administration and is available for commercial use.…”

“…If peak bicarbonate concentration, rather than total bicarbonate output or volume, is used as the primary parameter, no perfusion markers are generally required as the concentration of bicarbonate should not vary tremendously 40 ,. 41 In this situation, a double‐lumen tube with one port in the antrum and the second port in the duodenum is adequate.…”

“…Furthermore, the gastroduodenal aspiration tubes used for pancreatic fluid collection are no longer being manufactured. We previously reported our observations with a less cumbersome, purely endoscopic collection method for pancreatic function testing [4,5].…”
