Systematic Evaluation of Systemic Right Ventricular Function

Abstract: Background: The right ventricle serves as the subaortic systemic ventricle (sysRV) in patients with congenitally corrected transposition of the great arteries (ccTGA) and in patients with transposition of the great arteries (TGA) surgically repaired by an atrial switch. SysRV can lead to late complications, primarily heart failure, significant regurgitation of the systemic atrioventricular (AV) valve, and ventricular arrhythmias with sudden cardiac death. We sought to investigate the value of 2D- and 3D-echoca… Show more

“…In this case, the patient had just 1 ostia located in the right aortic sinus. The right ventricular coronary arterial supply may be inadequate for long-term ventricular function at systemic pressures, and that silent progressive irreversible ischemia of the right ventricular myocardium may make an important contribution to the development of right ventricular dysfunction and symptoms ( 6 ).…”

“…Possibly, a more complex echocardiographic assessment including longitudinal function parameters, ie. global longitudinal strain and 3D volumetric function parameters should be employed into comprehensive evaluation of the patients with the systemic right ventricle (27).…”

“…Given that SRV dysfunction is a key factor associated with morbidity and mortality, the accurate evaluation of SRV performance is crucial ( 1 ). Accuracy of conventional echocardiographic parameters is suboptimal: neither tricuspid annular plane systolic excursion (TAPSE) nor S′ correlated with CMR-derived SRV EF, whereas FAC showed only moderate correlation ( 2 ). Conversely, 3DE provides more granular understanding of the role of different components of the SRV systolic function ( 3 ).…”
