Systematic Review of Intraoperative Anesthesia Handoffs and Handoff Tools

Abstract: Intraoperative handoffs between anesthesia clinicians are critical for care continuity. However, such handoffs pose a significant threat to patient safety. This systematic review synthesizes the empirical evidence on the (a) effect of intraoperative handoffs on outcomes and (b) effect of intraoperative handoff tools on outcomes. All studies on intraoperative handoffs and handoff tools published until September 2019, in any study setting and population, and with no prespecified criteria on the type of compariso… Show more

“…For example, using peripheral nerve blocks for primary hip and knee arthroplasty was associated with significantly lower rates of postoperative complications [ 29 ]. Moreover, intraoperative anesthesia handovers [ 30 ▪ , 31 ] as well as staffing ratios of supervising anesthesiologists [ 32 ▪ ] were related to higher rates of postoperative mortality and complications. Hence, monitoring overall postoperative complications and mortality is crucial.…”

“…In addition, professional societies can support research and practice development aimed at advancing indicator contents. New modifiable risk factors emerging from research may be evaluated as quality indicators (e.g., anesthesia handovers [ 30 ▪ , 31 ], staffing [ 32 ▪ ]). In terms of value [ 65 , 66 ], the perspectives of patients and their caregivers should be included in indicator research [ 15 ▪▪ ] and development [ 98 ].…”

“…Psychological safety is the level of interpersonal risk team members are willing to take, and is usually established by leadership and is a key factor in effective teamwork. Work on specific healthcare team processes, such as care transitions and handoffs, has focused heavily on communication and shared mental models (Abraham et al, 2021; Gisick et al, 2018; Keebler et al, 2016). Even though the items listed in Table 3 may be too much to include in one particular study, they tend to be the super-ordinate set that can act as a starting point for healthcare teams research.…”

“…Previous studies that evaluated anesthesia handovers were largely retrospective analyses focused on a specific patient population (eg, cardiac surgery) . Overall results are conflicting because currently available evidence supports and refutes the assertion that anesthesia handovers are associated with increased perioperative complications. Thus, the core question remains whether frequent handovers of care by anesthesia professionals are beneficial, detrimental, or unrelated to surgical patient safety, overall quality of care, and relevant patient outcomes.…”
