Systematic review of the impact of endoscopic ultrasound on the management of patients with esophageal cancer

Abstract: Two studies provided evidence of a change in patient management following EUS for staging esophageal cancer, a higher level of evidence for a clinical benefit than can be obtained from accuracy studies alone. This evidence contributed to a recommendation for public funding of EUS in staging esophageal cancer in Australia.

“…[4][5][6][7][8] In concert with the rising burden of gastrointestinal and hepatobiliary conditions in Canada and elsewhere, [9][10][11][12] the applications of interventional endoscopic ultrasonography continue to increase and currently include drainage of pancreatic fluid collection, biliary drainage, celiac plexus nerve blockade and radiologic placement of fiducial markers. 13 Multiple investigations have shown that endoscopic ultrasound evaluation can alter patient management in up to 74% of cases, including those of gallstone disease or pancreatitis as well as esophageal, pancreatic, stomach and rectal cancers. For most patients, endoscopic ultrasound evaluation leads to less invasive and costly treatments.…”

“…For most patients, endoscopic ultrasound evaluation leads to less invasive and costly treatments. [13][14][15][16] Single-and multicentre studies have shown an increase in the application of endoscopic ultrasonography over time; 17,18 however, these findings may not be generalizable to broader populations. Furthermore, little is known about regional Regional differences in use of endoscopic ultrasonography in Ontario: a population-based retrospective cohort study trends in use.…”

“…Again, however, there are few studies examining the effect of EUS staging on subsequent management. In their systematic review, Dyer et al 105 acknowledged that drawing conclusions from observational studies, rather than RCTs, was open to bias, but estimated that EUS appeared to change management in 24-29% of patients. Two retrospective, 106,107 and therefore suspect, studies examined the effect of EUS staging on patient survival.…”
