Systemic cobaltism manifesting as oral mucosal discoloration and metallic gustation after metal-on-metal hip resurfacing

Abstract: Systemic cobaltism is a debilitating complication of metal-on-metal (MoM) arthroplasty. In this report, we review a case of a 54-year-old female with metallosis from a MoM hip resurfacing and varying degrees of black discoloration of her tongue and metallic gustation as a result of systemic cobaltism. After explanting the metal components, thorough debridement, and conversion to ceramic-on-polyethylene arthroplasty, the patient’s oral mucosal discoloration and metallic gustation resolved. This represents the f… Show more

“…b Treatments of arthroplasty-related metallosis. A schematic diagram showing the current treatments of metallosis and the result of no treatment adverse reactions and complications, leading to prosthetic loosening, osteolysis, and complete failure of the implant [13,19,26,42,47,51]. The current treatment guidelines to avoid further shredding of the implant particles into tissues are revision surgery (Fig.…”

“…In contrast to the result of a large head on total hip arthroplasty (THA), MoM hip resurfacing with a smaller femoral head size has increased risk of mechanical deterioration and edge loading with high release of metal particles. Mobile-bearing knees has also been associated with an increased rate of metallosis due to increased release of smaller particles and granular debris compared to fixed bearing knees [6,19].…”
