Systemic inflammatory dynamics during chemoradiotherapy predict response, relapse, metastasis, and survival in esophageal carcinoma

Abstract: Background and ObjectivesLymphopenia associated with chemoradiotherapy predicts prognosis in esophageal carcinoma. The purpose of our study was to evaluate alterations in hematologic measures of inflammation during chemoradiation.MethodsWe performed an observational study evaluating adults treated with chemoradiation in the neoadjuvant or definitive setting for stage II‐III esophageal carcinoma. Multivariable logistic regression evaluated predictors of pathologic response. Survival was analyzed by time‐varying… Show more

“…Meanwhile, emerging evidence indicates that host inflammation relates to cancer pathophysiology and progression 10‐13 . Systemic inflammatory markers (SIMs), which derive from the peripheral blood, such as neutrophil‐to‐lymphocyte ratio (NLR), platelet‐to‐lymphocyte ratio (PLR), and lymphocyte‐to‐monocyte ratio (LMR), have been extensively investigated for diagnostic and prognostic value in a variety of solid tumors including thyroid carcinoma 14‐23 . Integrated indicators such as neutrophil‐platelet‐lymphocyte ratio (NPLR) and neutrophil‐monocyte‐lymphocyte ratio (NMLR) have been developed.…”

“…Neutrophil-to-lymphocyte ratio (NLR) is a novel predictive biomarker that was investigated in three recent studies [ 88 , 89 , 90 ]. The studies were unanimous in demonstrating that NLR was correlated with pCR.…”

“…and lymphopenia. While high counts of activated neutrophils are known to inhibit cytolytic activity of T-cells and natural killer cells (27), systemic lymphopenia may reflect a decreased presence of lymphocytes in the tumor microenvironment representing a reduced specific host immune response to the tumor (28). Tumor infiltrating lymphocytes are part of the specific host immune response contributing to therapy-mediated tumor control in solid tumors, possibly triggered by the recognition of tumor neoantigens unmasked by the effects of radiochemotherapy (29-31).…”
