From 2003 to 2008 a multicentre research project, systems therapy methods in acute psychiatry (SYMPA), attempted to establish structured in-patient treatment based on systemic interventions in German nonuniversity psychiatric hospitals. Utilising multi-professional family systems training for team members in six in-patient wards, its main goal was to improve the involvement of familial and non-familial important others. This follow-up study explores the sustainability of family systems psychiatry by evaluating the frequency of the utilisation of four core systemic interventions; elements of a systemic organizational culture in psychiatry; and institutional barriers to this new approach. The degree of implementation was high to moderate, depending on the intervention. More intense communication developed among staff of different professions and the status of nursing staff improved. Organizational barriers to implementation included the ambiguous role of junior doctors in teams, continuously understaffed wards and frequent over-occupancy.

“…Von 2003 -2005 wurden 6 komplette Stationsteams (Pflegekräfte, Psychologen, Ärzte, Sozialarbeiter) parallel zur laufenden klinischen Tätigkeit hierarchieübergreifend weitergebildet [22]. Hinsichtlich des Therapieergebnisses der Patienten nach erfolgter Weiterbildung fand sich kein signifikanter Unterschied, obwohl in der Brief Psychiatric Rating Scale tendenziell höhere Effektstärken für die Interventionsgruppe beobachtet wurden [23] Aufgrund der inakzeptablen Ergebnisse in der Faktorenanalyse und bezüglich der internen Konsistenz in der letzten deutschen Validierungsstudie [32] wurden in dieser Untersuchung die Items der optional konzipierten Skala "Betroffenheit" aus dem Fragebogen entfernt. Für die übrigen 3 Skalen zeigten sich zufriedenstellende Ergebnisse der Faktorenstruktur und Konstruktvalidität.…”

The involvement of relatives is seen as an important element of treatment in crisis resolution and in home treatment teams. The aim of the study was to investigate the effects of involving relatives in treatment during a crisis situation. The outcomes for patients from a relatives-involved group (RIG) and relatives-not-involved group (RNIG) were compared. Relatives were provided with general information about the service and assessments, psycho-education and, in some cases, additional family interventions. Relatives were involved in the treatment of 168 of 263 patients (64 per cent). There were no significant differences between the RIG and RNIG in terms of mean scores for BSI at T12. The involvement of relatives was achieved in the majority of cases. No link with outcomes was found. Since involvement of relatives was not randomized and the number and duration of contacts with relatives were limited, these findings should be interpreted cautiously.Practitioner points • Using a structured motivational model, it is possible to involve relatives in the treatment of psychiatric crisis patients in the majority of cases • There is no reason to be reticent about involving relatives in treatment, even if patients seem to be reluctant at first sight • Living alone was in this study identified as a negative predictor of family involvement
