Systolic Time Ratio by Impedance Cardiography to Distinguish Preserved vs Impaired Left Ventricular Systolic Function in Heart Failure

Abstract: Left ventricular ejection fraction (EF) is used to assess patients with heart failure (HF); however, frequent measurements are not cost-effective. Impedance cardiography (ICG) is a low-cost, noninvasive test that measures systolic time intervals and may be a method for detecting impaired vs intact EF. This study evaluated the relationship between EF by echocardiography or gated nuclear ventriculography and systolic time ratio (STR) by ICG in outpatients with chronic HF. A retrospective chart review identified … Show more

“…In healthy subjects, hyperoxaemia increases the pulmonary blood flow and decreases the HR, but CO is only slightly changed [22]. According to a study by Thompson et al, STR (PEP/LVET) measurement can be a reliable method for evaluation of ventricular systolic function, and differentiate patients with preserved and impaired ventricular systolic function [23]. In observations in the presented study, STR was elevated in the sitting position, which indicates impaired ventricular systolic function.…”

“…Evaluation of left ventricular function was the subject in previous studies with ICG but none of the parameters we found associated with EF was previously regarded as a determinant of left ventricular systolic dysfunction. Systolic time ratio, however, was able to distinguish preserved from impaired EF [17]. Also, a close correlation between systolic time ratio and changes in EF was observed before and after treatment in patients with heart failure [18].…”

“…These early glycation products undergo further complex reaction, such as rearrangement, dehydration and condensation, to become irreversibly cross-linked, heterogeneous fluorescent derivatives, termed advanced glycation end products (AGEs) [1]. The formation and accumulation of AGEs have been known to progress under oxidative stress conditions such as diabetes and insulin resistance [1,2]. There is a growing body of evidence that AGEs are implicated in a variety of disorders such as vascular complications of diabetes, Alzheimer's disease and insulin resistance [1][2][3][4].…”

“…Biompedance is a method of body composition analysis studied in HF in different ways such as impedance cardiography [2] or as a home monitoring method in telemedicine systems [3], but little data exist about the analysis of the components of the impedance vector (BIVA-Bioimpedance Vector Analysis-) in HF. The result of a measurement using bioelectrical impedance is a vector (Z vector) in which the opposition of an alternating current flow through intra-and extracellular ionic solutions is resistance (R), and the capacitance of biological tissues, is reactance (Xc).…”
