Takotsubo cardiomyopathy complicated by cardiac tamponade

Abstract: We describe a 76-year-old patient with takotsubo cardiomyopathy complicated by cardiac tamponade. Pericardial effusion in takotsubo cardiomyopthy is common but a cardiac tamponade is very rare. The use of anticoagulants may increase the risk of pericardial effusion and should be considered with care.

“…Pericardial effusion is rarely described in TTS [ 61 , 62 ] and is more frequently associated with an inflammatory disease of the myocardium and/or pericardium. If pericardial effusion is detected, further investigation by cardiac magnetic resonance (CMR) should be performed to investigate the differential diagnosis of myocarditis [ 17 , 26 ].…”

“…Large systemic registries or multicentre trials [2022] do not only increase awareness, they also result in a better understanding of the demographics and ultimately also the treatment of takotsubo cardiomyopathy. Furthermore, case reports concerning a new clinical presentation of takotsubo cardiomyopathy are more often published [23, 24]. …”

“…In most of these cases, non-steroidal antiinflammatory agents were effective, and no additional treatment was needed. Two cases, in which pericardiocentesis was applied to improve the cardiac tamponade, have been reported [1,2]. The pericardial effusion in these cases was hemorrhagic, derived from cardiac rupture, and no studies on cardiac tamponade complicated with TC-related inflammatory effusion have been reported.…”

“…Pericardial effusion is another well-known complication of TC, but it rarely affects the hemodynamic status. Two cases of cardiac tamponade subsequent to TC have been reported [1,2]. The pericardial effusion in these cases was hemorrhagic and caused by ventricular rupture.…”
