Tandem hemodialysis and plasma exchange

Abstract: The combination of hemodialysis and plasma exchange as one tandem procedure was first described in 1999 by Siami et al. (ASAIO J 45:229-233), but larger pediatric case series were not described until 2012. Even in adults, there are only limited case series. If performed in sequence, up to 8 h of treatment time may be required. With the use of the tandem procedure in stable patients, the same procedures can be completed during the same time as a routine hemodialysis, which is more convenient for patients and ma… Show more

“…Case series from adult patients with immune-mediated disease indeed indicate a time-saving effect (3)(4)(5)(6). In a recent survey, 23 of 25 pediatric nephrology centers confirmed the demand for combined PE/HD (cPE/HD) treatment, mainly in children with atypical hemolytic uremic syndrome (HUS) and vasculitis, but also in children with renal graft rejection and recurrent FSGS (7). A case series of eight children in an Italian pediatric center suggests the feasibility of tandem dialysis (8).…”

“…1). Patients who have received tandem TPE with intermittent hemodialysis (HD) in both adults and children have been described [3][4][5][6][7][8][9][10][11]. Yorgin et al described the use of plasmapheresis during CKRT in 2000 [12].…”

“…14 Other studies have reported on tandem TPE and hemodialysis (HD) in adults and children. [15][16][17][18][19][20][21][22] Unlike in the mentioned studies, we have used easily available Yconnectors to establish parallel or sequential (seriesparallel) connections at the vascular catheter or in the CKRT return/efferent line, respectively. In addition, we also report in our study on the technical and metabolic parameters during TPE-CKRT.…”
