Taxane‐based chemoirradiation for organ preservation with locally advanced head and neck cancer: Results of a phase II multi‐institutional trial

Abstract: There were no significant differences in relapse-free survival or organ preservation rates between concurrent regimens. Platinum and paclitaxel-based CCR is feasible after ICT and provides a high rate of organ preservation. Substitution of concurrent cisplatin to weekly carboplatin with paclitaxel and radiation has an improved toxicity profile. The ease of administration and low toxicity make this a regimen that is practical for use in the community setting.

“…Although follow‐up is brief, this CR has been sustained, with 13 patients being disease free for over 2 years after completing therapy. Similar protocols of induction chemotherapy followed by concurrent chemoradiation in patients with head and neck cancer have shown overall response rates ranging from 80% to 90% with CR rates ranging from 40% to 90% 3,7 . Although these trials have larger numbers and are not base of tongue specific, the results provided here are similar with an overall response rate and CR rate of 94%.…”

“…Paclitaxel -representative of the second generation chemotherapy -has been used in SCCHN single-agent [7,14] or platinum-based radiochemotherapy protocols [2,6,8,15,22,25]. Current treatment strategies often use hyperfractionated or hyperfractionated-accelerated radiotherapy protocols to enhance efficacy.…”

“…This percentage is very much higher than that of other studies and, probably, would have had a negative impact on the response rate and OS. Secondly, the concurrent schedule differs signifi cantly from trials evaluating concurrent taxanes and radiotherapy [11,12,14,[21][22][23][24]. For example, Matsumoto et al published the results of a study in 25 patients with advanced H&N treated with weekly docetaxel (10 mg/m 2 ) and hyperfractionation radiotherapy (72 Gy at 1.2 Gy twice daily).…”
