Technical developments and a team approach leads to an improved outcome: Lessons learnt implementing laparoscopic splenectomy

Abstract: A reduction in conversion rates, operating time, morbidity and length of stay was realized during the learning curve of implementing laparoscopic splenectomy by adopting a standardized technique. This technique involved hilar dissection using the ultrasonic shears, two experienced laparoscopic surgeons performing the surgery, dedicated equipment and trained staff using the checklist approach. We recommend such a standardized technique in performing laparoscopic splenectomy.

“…Conversely, MMC has a greater referral base for haematological problems and splenectomy for haematological indications has been a specific interest at MMC. 12 On univariable analysis, several factors were significantly associated with a lower likelihood of certain postsplenectomy interventions, as shown in Table 4. For every 10-year increase in age, patients were 36% less likely to receive long-term antibiotics (odds ratio (OR) 0.64; 95% confidence interval (CI) 0.54-0.80; P < 0.001).…”

“…Because DDH has a greater exposure to trauma patients, there is a significant difference in the indications for traumatic splenectomy at this hospital. Conversely, MMC has a greater referral base for haematological problems and splenectomy for haematological indications has been a specific interest at MMC 12 …”

“…Furthermore, the presence of a dedicated theatre team trained in laparoscopic techniques can positively affect surgical outcomes. 14,15 Finally, examining the 'root cause' of conversion, as done previously for laparoscopic cholecystectomy, is an important step in assessing the safety of a technique and determining patient suitability. 16 Our results show that as experience in VATS increases over time, conversion rates secondary to technical reasons decrease.…”

“…The article in this edition of the Journal, ‘Technical developments and a team approach leads to an improved outcome: Lessons learnt implementing laparoscopic splenectomy’ by Chan et al . provides further evidence of the advantages of laparoscopic splenectomy 5 . Apart from pointing out what has become very evident over the last 6 or 7 years, this paper highlights one of the vital ingredients in a successful laparoscopic surgical practice, namely the establishment of a surgical team dedicated to laparoscopic techniques.…”
