Background and objectives: This study was taken up to know the utility of application of Direct Immunofluorescence on skin biopsies with particular reference to Autoimmune Bullous disorders.

The objectives of the present study was to detect specific patterns of DIF in different Autoimmune bullous disorders, to supplement clinical and histopathological features, and hence for confirmatory diagnosis of these disorders.

Methods: DIF using fluorescent labelled antibodies – IgG, IgM, IgA and C3 - was carried out in 60 clinically suspected cases of Autoimmune bullous disorders referred to Department of Pathology, KIMS Hospital and Research Center, Bangalore, over a period of 18 months from January 2011 to July 2012.

Results: Out of 60 cases analysed, 39 cases were given a final diagnosis as Autoimmune bullous disorder based on clinical, histopathological and DIF findings. DIF positivity was observed in 34 cases. DIF findings correlated with clinical and histopathological findings in 37 cases (94.8%).

Interpretation and Conclusion: In this study we found out that DIF serves as a simple, highly sensitive, cost-effective and hence gold standard test for Autoimmune bullous disorders. The technique is essential to supplement clinical findings and histopathology in the diagnosis of these Immunobullous disorders.

SUMMARY:An alternative superovulator to replace clomiphene citrate is needed as clomiphene citrate is associated with low pregnancy rates. Anastrozole is an effective superovulator, but it has not been well researched. In order to determine the effectiveness of anastrozole as a superovulator and to compare it with clomiphene citrate in similar situations, this study ascertained the effects of these drugs on the expression of the focal adhesion proteins, vinculin and integrin β5, which are uterine receptivity markers, in the uterine epithelial cells of day 1 and day 6 pregnant Wistar rats. The results show that vinculin and integrin β5 are co-localized at the base of the uterine epithelium at day 1 of pregnancy whereas at day 6, they disassemble from the basal focal adhesions and co-localize and significantly increase their expression apically (p≤0.0001). Moreover, there is a significant difference in the protein expression levels of vinculin and integrin β5 in uterine luminal epithelial cells between untreated (control) and chlomiphene citrate treated rats (p≤0.0001), anastrozole and chlomiphene citrate treated rats at day 6 (p≤0.0001) suggesting the interpretation that anastrozole seems to enhance their expression in order to perhaps assist in the implantation process of the blastocyst. The immunofluorescence experiments agree with the vinculin and integrin β5 gene expression findings in which at day 6 of pregnancy, vinculin and integrin β5 gene expression are significantly upregulated in uterine luminal epithelial cells in the anastrozole treated group relative to the calibrator sample (p≤0.0001). These findings suggest that anastrozole is implantation friendly.

Dermatological diseases with underlying immunological pathosis exhibiting similar clinical manifestations have challenged even the veteran dermatologists in rendering diagnosis clinically, necessitating study of the skin lesions with immunofluroscence. The technique is similar to immunochemistry where instead of enzymes antibodies are labelled with a fluorescent dye4. It probes the study of the cell and molecular biology. Immunofluorescence permit early diagnosis, treatment and subsequent monitoring of disease activity in patient with potentially life threatening disease. It is widely used in the field of vesiculo-bullous lesions and other related oral dermatological disorders by demonstrating antibodies either in biopsy tissue or in the serum separated from blood collected from the patients.
