Telerobotic Manipulation of the Brachial Plexus

Abstract: Robot-assisted surgery allows performance of high-dexterity surgical operations with the help of robotic arms and it improves the surgery due to tremor filtration, motion scaling, and ergonomics. The benefit of using the robot on microsurgery was reached, but its entire potential was not realized because the instruments used on those first experimental and clinical cases were not specifically designed for microsurgery.

“…Epineural nerve repair using robotic assistance has been shown to be technically feasible in animal models, with the benefits of reduced physiological tremor and improved vision of the surgical field noted ( 35 ). Nerve harvest has also been demonstrated to be feasible in cadaveric and animal models ( 35 , 37 ).…”

“…The interest of the endoscopic technique is a share in the cosmetic aspect, and second in respect of the perineural environment. 20,21 Scar 1 cm perineural fibrosis causes less scarring than 15 cm, as in the series of Leechavengvongs et al 2 Reasons for failure of the endoscopic technique could be explained by the number of incisions, absence of sensory feedback, and limited dissection space. In upper limb surgery, the limited working space with multiple portals can be responsible for mechanical conflict between the arms of the robot.…”

“…Orthopaedic surgery has just recently added this technology for open brachial plexus reconstruction, 14 as well as microsurgical nerve procedures. 13,15,16 The advantages of robotic surgery are as follows: (1) Tremor filtration makes the surgical procedure stable (6) Ergonomics is improved because the surgeon is out of the surgical field, seated in a comfortable position.…”
