PurposeThis study investigated the correlation between the relative afferent pupillary defect (RAPD) and retinal nerve fiber layer thickness (RNFLT) in optic neuropathy.MethodsRAPD assessment was performed using a log unit neutral density filter bar. Spectral domain optical coherence tomography RTVue-100 (Optovue) was used to examine the subjects. The optic nerve head pattern (ONH) was subdivided and identified for the purpose of the study into circumpapillary RNFLT (cpRNFLT) and peripheral circumpapillary RNFLT (pcpRNFLT). The cpRNFLT, pcpRNFLT and ganglion cell complex (GCC) parameters were analyzed.ResultsEighteen females and twenty three males with asymmetric optic neuropathy and a RAPD participated. Thirty-three subjects had glaucoma and eight had optic neuropathy other than glaucoma. Significant correlations (p < 0.02) were obtained for the RAPD and the percentage difference loss of the GCC and RNFLT parameters. The grouped mean percentage difference loss for RNFLT was significantly different from that of the GCC (p < 0.001). At a 0.6 log unit RAPD, the average mean percentage difference loss was 23% for the CRNFLT, 15% for the GCC, 12% for the global loss volume percentage and 6% for the focal loss volume percentage (FLV%).ConclusionsSignificant correlations between RNFLT loss for cpRNFLT, pcpRNFLT and GCC parameters with RAPD were observed. Approximately a 35% higher sensitivity was obtained using GCC compared to CRNFL parameters. The expected change in GCC average for every 0.3 log unit increment was approximately 8.49 μm. The FLV% corresponded more sensitively to a RAPD but appeared to be influenced by disease severity.

Deep learning (DL) models are very effective on many computer vision problems and increasingly used in critical applications. They are also inherently black box. A number of methods exist to generate image-wise explanations that allow practitioners to understand and verify model predictions for a given image. Beyond that, it would be desirable to validate that a DL model generally works in a sensible way, i.e. consistent with domain knowledge and not relying on undesirable data artefacts. For this purpose, the model needs to be explained globally. In this work, we focus on image modalities that are naturally aligned such that each pixel position represents a similar relative position on the imaged object, as is common in medical imaging. We propose the pixel-wise aggregation of image-wise explanations as a simple method to obtain label-wise and overall global explanations. These can then be used for model validation, knowledge discovery, and as an efficient way to communicate qualitative conclusions drawn from inspecting image-wise explanations. We further propose Progressive Erasing Plus Progressive Restoration (PEPPR) as a method to quantitatively validate that these global explanations are faithful to how the model makes its predictions. We then apply these methods to ultra-widefield retinal images, a naturally aligned modality. We find that the global explanations are consistent with domain knowledge and faithfully reflect the model's workings.Preprint. Under review.

Purpose: To evaluate the central visual field by microperimetry (MP), in early glaucoma. Methods: Consecutive perimetrically experienced patients with a single nasal step or arcuate scotoma and 14 control eyes underwent MP. Retinal sensitivity on MP was mapped for frequency and depth of loss in the central 10° around fixation. Results: Twenty-one eyes had a single nasal step and 19 eyes with single arcuate scotoma on standard automated perimetry (SAP), with central 10° being normal on 30–2 and 10–2 perimetry. The average mean sensitivity on MP, in glaucomatous and control eyes was 11.8 ± 3.9 dB and 16.6 ± 1.2 dB, respectively, P = 0.0004. The average mean defect on MP-1, in glaucomatous and control eyes was - 6.5 ± 2.0 dB and - 3.0 ± 1.2 Db, respectively, P = 0.05. The corresponding retinal hemisphere showed significant defects in MP. In eyes with single nasal steps, an absolute scotoma was seen in 14–28% of eyes 8–10° off fixation, moderate to mild defects were seen in 10–52% eyes, and 10% eyes showed involvement up to 4° from the fixation. Eyes with arcuate scotoma had an absolute scotoma on MP in 95% of eyes, 6–10° from fixation, with extension up to 2° from fixation in 21%. In glaucomatous eyes, the normal hemisphere on SAP showed a mild defect on MP in 43%. Control eyes did not show any defect in SAP or MP. Conclusion: A significant loss of central retinal sensitivity is recorded on MP in early glaucomatous neuropathy as compared to SAP. Paramacular absolute defects were seen at 6–10° from fixation.
