Ten‐year follow‐up of dental implants used for immediate loading in the edentulous mandible: A prospective clinical study

Abstract: Immediate loading of 5 fluoride-modified dental implants with a fixed prosthetic rehabilitation is a predictable and reliable treatment in the edentulous mandible, based on a 100% implant survival and limited peri-implant bone loss. Implants used for immediate loading in the edentulous mandible who are showing early bone loss may be at higher risk to develop peri-implantitis.

“…Vervaeke et al showed less marginal bone loss in fluoridated surface implant group than in TiO blasted implants, when used in edentulous mandible . Windael et al presented a survival rate in the mandible of 100% after 10 years and low bone marginal loss 0.49 mm (SD 1.08, range 0‐7), in predominantly bone quantity of A and B . However, the results of our long‐term study suggest that implant surface modification might not compensate for compromised host bone.…”

“…Recent consensus statements have concluded that immediate loading with fixed prostheses in carefully chosen edentulous patients results in similar implant and prosthesis survival rates when compared with early and conventional loading . However, most studies document short‐ or medium‐term follow‐up with 1, 3, and 5 year outcomes . There is a paucity of long‐term data of 10 years or more documenting immediate loading protocols in compromised bone volume while avoiding bone augmentation.…”

“…In line with altered components and their impact on the osseointegration process, surgical procedures were adjusted to meet the requirements for simpler and quicker treatment. Thus, immediate implant placement after extraction was one way to reduce the treatment period, as well as one‐stage implant surgery with either early or immediate nonfunctional/functional loading . Implant concurrently placed with various bone augmentation interventions was another means for time reduction .…”

“…Dental implants are a modern substitute for dentures for patients with missing teeth. Studies have shown that dental implants can remain esthetically and functionally stable for prolonged durations in a manner similar to natural teeth. Periodontal and peri‐implant disease are inflammatory conditions associated with the soft (gingivae) and hard (alveolar bone) tissues surrounding teeth and dental implants, respectively.…”
