Background There is a lack of long‐term evaluations of immediately loaded implants in patients with compromised maxillary bone. Purpose To evaluate long‐term survival and success of immediately loaded implants in subjects with poor maxillary bone quality and volume. Material and methods Follow‐up was performed on subjects who received six implants loaded within 24 hours with screw‐retained fixed prostheses. Twenty‐five subjects with limited bone (Lekholm and Zarb, quantity 3 and 4, and quality C and D) were included in the study. Nineteen participated in the radiographic examination and of these 17 participated in the clinical examination at the last visit. Evaluations of marginal bone loss were performed at 1, 3, 5, and 8‐11 years. The last clinical examination included removal of the prosthesis followed by registration of: plaque, pus, pocket depth, bleeding upon probing, mobility, and percussion testing. Results The mean follow‐up was 9 years and 2 months (101‐131 months) after surgery and showed a cumulative implant survival rate of 81.9% and success rate of 74.7%. Mean marginal bone loss was 1.29 mm (SD 2.47 mm range 0‐11 mm) with a mean pocket depth of 3.1 mm (SD 2.4 mm, range 1.5‐13.5 mm). 42.5% of the implants showed plaque retention and 72.2% showed bleeding on probing. Conclusion This nonaugmenting immediate loaded implant protocol for maxillary edentulous patients is a satisfactory solution for selected patients.

Background A new implant design was launched in 2015 with internal connection allowing for placement of abutments with angulated screw channels. Such devices may eliminate the use of nonauthentic components, which may present with poor fit and jeopardize the construction. It may also eliminate the use of cemented single‐crowns with the risk for excessive cement to cause marginal bone destruction. Purpose To evaluate the outcome of a straight‐walled implant with conical connection (NobelParallel CC), when used in the single‐tooth situation of the anterior maxilla. Further, to elucidate the need for an abutment with angulated screw channel (ASC). Materials and Methods Patients referred for both implant surgery and prosthetics to rehabilitate missing single maxillary incisors/canines. Implants were placed according to a two‐staged surgical procedure. Patients were followed up to delivery of prosthetic crowns with the purpose to extend the study up to 1‐, 3‐, and 5‐year follow up. Registrations of implant survival, marginal bone level measurements, hard/soft tissue complications, and the incidence of ASC to avoid the use of nonauthentic components or crown cementation, were performed at the end of the study. Results A total of 47 patients were included in the current investigation. One patient deceased during the study period. In all 51 implants were inserted, of which 14 implant sites were bone augmented either prior to implant placement or in connection with the insertion procedure. Further, four remaining teeth were extracted in the region of interest during implant placement. One implant presented with infection and mobility at abutment connection and was consequently removed. Implant survival rate was 98.0%. Mean marginal bone level at crown placement was 0.41 mm (SD = 0.45 mm) and the corresponding measurements at 1 year for 22 implants revealed a marginal bone resorption of 0.41 mm (SD = 0.36 mm). Abutments with angulated screw channels were used in 42/49 sites. Conclusion The NobelParallel CC implant seemed to work well together with the ASC abutment. No major complications were noted.

The aim of this review was to assess the contribution of herpesviruses in the subgingival oral biofilm in the progression of periodontal and peri-implant diseases in systemically healthy individuals. The literature review was customized to summarize the pertinent information for the following reasons: (1) A systematic review regarding the role of herpesviruses in the etiopathogenesis of periodontal disease has recently been published; and (2) a limited number of studies have assessed the association of herpesviruses with peri-implant diseases. To date, five observational studies have assessed the presence of herpesviruses in the subgingival oral biofilm of individuals with peri-implant diseases. In these studies, dental implants were in place for up to approximately 8 years. In two studies, human cytomegalovirus (HCMV) was more often isolated from the peri-implant sulci of sites with than without peri-implantitis. In one study, a low prevalence of HCMV compared with Epstein-Barr virus (EBV) was associated with the progression of peri-implantitis. In previous studies, the presence of EBV in the subgingival oral biofilm was associated with the onset of peri-implantitis and peri-implant mucositis, respectively. Major limitations of the studies assessed were the absence of blinding and lack of power analysis for sample size estimation. In conclusion, the presence of herpesviruses in the periodontal and peri-implant subgingival oral biofilm is an indicator of periodontal and peri-implant diseases in systemically healthy individuals; however, further studies with a statistically justified sample-size are needed to understand and refine this association.
