We compared the accuracy of direct susceptibility testing (DST) with conventional antimicrobial susceptibility testing (AST), both using disk diffusion, on clinical samples. A total of 123 clinical samples (respiratory tract samples, urine, vaginal and abdominal abscess discharges, bile fluid and a haematoma punctate) were selected on various indications; direct inoculation on Mueller–Hinton agar and antibiotic paper disks were applied. In parallel, standard culture, identification and AST on the colonies grown overnight was executed. Both AST and DST were interpreted after identification of the isolates. The results from both AST and DST for 11 antibiotics tested on 97 samples with Gram-negative rods showed 93.4 % total agreement, 1.6 % minor discordances, 4.6 % major discordances and 0.4 % very major discordances. Analysing the discordant results, DST predominantly resulted in more resistant isolates than AST. This was mostly due to the presence of resistant mutants or an additional isolate. The remaining discordances were seen for isolates with inhibition zones close to the clinical breakpoint. For the 26 samples yielding staphylococci, a total agreement of 100 % was observed for the nine antibiotics tested. Overall, the highest percentage of discordant results occurred for the β-lactam antibiotics amoxicillin–clavulanate (13.4 %) and cefuroxime (12.4 %). When used selectively and interpreted carefully, DST on clinical samples is potentially very useful in the management of critically ill patients, as the time to results is shortened by approximately 24 h. However, we recommend to communicate results with reservations and confirm by conventional AST.

The current culture-based approach for the diagnosis of bloodstreams infection is incommensurate with timely treatment and curbing the prevalence of multi-drug resistant organisms (MDROs) due to its long time-to-result. Bloodstream infections typically involve extremely low (e.g., <10 colony-forming unit (CFU)/mL) bacterial concentrations that require a labor-intensive process and as much as 72 hours to yield a diagnosis. Here, we demonstrate a culture-free approach to achieve rapid diagnosis of bloodstream infections. An immuno-detection platform with intrinsic signal current amplification was developed for the ultrasensitive, rapid detection, identification (ID) and antibiotic susceptibility testing (AST) of infections. With its capability of monitoring short-term (1–2 hours) bacterial growth in blood, the platform is able to provide 84-minute simultaneous detection and ID in blood samples below the 10 CFU/mL level and 204-minute AST. The susceptible-intermediate-resistant AST capacity was demonstrated.

APMIS 1999; 107:4374.A method for diagnosis of bacteriuria using semiquantitative culture in combination with direct susceptibility testing is described. Susceptibility testing with the breakpoint method was performed by means of a multiple inoculation technique. A simple biochemical typing on solid media of lactosefermenting Enterobacteriaceae strains and aesculin testing of Enterococcus strains were performed simultaneously. Staphylococcus and Streptococcus strains were tested for susceptibility using the conventional disc diffusion technique. The results of the direct susceptibility test and the disc diffusion test were compared in a study of 983 consecutively positive urine cultures: 6624 susceptibility tests from 882 specimens with one microorganism revealed complete agreement in 6343 cases (95.8%); the discrepant results were 15 very major errors (0.2'%0), 84 major errors (1.3%) and 182 minor errors (2.7%). The direct susceptibility test revealed 42 more cases of bacteriuria with enterococci in combination with Enterobacteriaceae strains compared to the conventional method. 59% of all positive urine cultures were completed, including the susceptibility test report, within 24 h after arriving at the laboratory. The diagnostic costs were approximately the same as for the conventional method.
