Terms used in patient safety related to medication: a literature review

Abstract: A multitude of terms and definitions are used in patient safety related to medication. This heterogeneity makes it difficult to compare the results among studies and to appreciate the true magnitude of the problem. Classifying and unifying the terminology is necessary to advance in patient safety strategies.

“…Our primary goal was to document the incidence of AEs in the practice of nuclear medicine using prospective data collection by nuclear medicine scientists, clear definitions of AEs, (16,17), and a known denominator (16,17). With this approach, we believe we have overcome the problem of underreporting of AEs because of the transient nature of these events, confusion in the terminology of AEs, anxiety about potential liability, the time to complete a report form, and the lack of relevant reporting forms (16)(17)(18)(19), although Cold pyp † , 99m Tc-sestamibi*, 99m Tc-tetrofosmin (2 patients)* *Judged as unlikely by study criteria, totaling 5 AEs; if 2 patients are noted as having had AEs, only one was deemed unlikely in this study.…”

“…These terms are often understood very differently depending on the individuals or professionals involved; their meaning also depends on the domain of use, whether this be clinical, regulatory, or even legal. 3 In addition, if the focus is on medication-related terms, confusion may arise from whether one is referring to the process of medication use, to the clinical outcomes of medication use, or to a combination of process and outcome. 3 As a pharmacist, I am first and foremost concerned with safety issues arising from medications that are given to my patients.…”

Both proper and improper uses of medications have the potential to create significant burdens on the health care system, because of a patient's admission to hospital from a drug-related problem, increased length of stay, or increased morbidity and mortality.…”

“…Even though definitions of DRMs vary in the literature (17)(18)(19), they include both outcomes below or beyond the optimal medication intended outcomes (20).…”
