Test–retest reliability of the Dyskinesia Impairment Scale: measuring dystonia and choreoathetosis in dyskinetic cerebral palsy

Abstract: Aim To assess test–retest reliability of the Dyskinesia Impairment Scale (DIS) in children and young adults with dyskinetic cerebral palsy (CP). Method Dystonia and choreoathetosis were assessed in 15 participants with dyskinetic CP (13 males, 2 females; age range 5–22y, mean 14y, SD 4y) using the DIS in two separate sessions over 7 days. Exclusion criteria were changes in muscle relaxant medication within the previous 3 months, orthopaedic or neurosurgical interventions within the previous year, and spinal fu… Show more

“…Thirdly, the DIS evaluates only the presence and severity of dystonia and choreoathetosis, whereas the BFM is constructed in such a way that the impact of dystonia on function is, by default, evaluated along with the severity of dystonia, which may complicate the interpretation of the results. Finally, the DIS has showed high inter-rater and test-retest reliability and validity in DCP [14,15].…”

“…In the first step towards developing the adapted DIMS protocol, the DIS was examined thoroughly for dystonia and choreoathetosis characteristics which would be relevant to be included in the DIMS [9,[14][15][16]. The DIS measures the presence and severity of dystonia and choreoathetosis over 12 body regions; i.e., central body (eyes, mouth, neck and trunk) and limbs (upper and lower, proximal and distal limbs) [14].…”

“…To date, presence and severity of dystonia and choreoathetosis are measured using the Dyskinesia Impairment Scale (DIS) [9,[14][15][16], a video-based tool which measures the movement disorders during requested but voluntary activities and rest postures. The DIS measures requested but voluntary activities in a controlled environment and may, therefore, not provide information about the occurrence of the movement disorders in a more real-life context, such as steering a powered wheelchair using a head/foot steering system.…”

“…standard error of measurement and smallest detectable change), should be used to determine if individual changes are due to treatment and not to measurement error. 25 Feasibility of the full DIS protocol is limited because many activities items of the DIS could not be performed in non-ambulatory children. Future research should elaborate the activity part of the DIS scale towards a reduction of activity items for this group of patients.…”
