Testing process errors and their harms and consequences reported from family medicine practices: a study of the American Academy of Family Physicians National Research Network

Abstract: Errors occur throughout the testing process, most commonly involving test implementation and reporting results to clinicians. While significant physical harm was rare, adverse consequences for patients were common. The higher prevalence of harm and adverse consequences for minority patients is a troubling disparity needing further investigation.

“…Six studies 12,17,21,30,31,41 used a combination of data sources. Reporting system were the most frequent data source: 15 studies (45%) 9,11,12,13,14,15,18,19,21,30,32,35,37,38,41 . The definition of adverse events differed, while the great majority of studies did not present a definition for these events.…”

“…[1][2][3][4][5][6][7][8][9] Test result management in an electronic health record (EHR)-enabled health care system is susceptible to both technological and non-technological vulnerabilities. 10,11 EHR-based test result notification systems are aimed at reducing delays in follow-up of abnormal test results, and likely represent an improvement over communication using paper-based methods.…”

“…Some authors have introduced the "pre-pre-" and "post-post-" analytical phases to identify activities associated with the initial selection of tests and with the interpretation by clinicians respectively, to differentiate them for the pure collection/transport activities (pre-analytical phase) and reporting (postanalytical phase) 13, 14 . There is some evidence that these steps are more error-prone than other pre-and postanalytical activities [13][14][15][16][17][18] .However, the definition and use of such terms is not universal. Indeed the definition of even basic terms such as pre-analytical, analytical and post-analytical can vary between authorities.…”
