Tetralogy of Fallot with coronary artery to pulmonary artery fistula

Abstract: What's already known about this topic?Coronary artery fistulas are rare anomalies with an incidence of 0.002% in the population. What does this study add?The association of tetralogy of Fallot, coronary artery fistulas, and agenesis of the ductus arteriosus has not been reported. The discrepancy between the narrow pulmonary annulus with thickened pulmonary valve and the large pulmonary trunk can be caused by extra blood supply through the large fistula.

“…6 We are also aware of two reports describing right coronary to pulmonary artery fistula. 8,9 A recent report established the diagnosis of RCA to pulmonary artery fistula prenatally, following which the pregnancy was terminated. 8 Although surgical correction remains the mainstay for treatment, nonsurgical management of a RCA to pulmonary artery fistula has been described, in which Trehan and colleagues used nonconventional coils to occlude the fistula preoperatively.…”

“…8,9 A recent report established the diagnosis of RCA to pulmonary artery fistula prenatally, following which the pregnancy was terminated. 8 Although surgical correction remains the mainstay for treatment, nonsurgical management of a RCA to pulmonary artery fistula has been described, in which Trehan and colleagues used nonconventional coils to occlude the fistula preoperatively. 9 In our case, transcatheter closure of the fistulous communication was not considered to be an option because the fistulous connection to the pulmonary artery originated from the main RCA, in contrast with the case of Trehan and colleagues, where the fistulous connection was from a branch of the RCA.…”

“…Tetralogy of Fallot (TOF) is the most frequently form of cyanotic heart defect or cyanotic congenital heart disease, and is the major cause of blue baby syndrome [ 1 ]. TOF is characterized by a misalignment of the conal septum, resulting in a rightward deviation of the aorta, which may lead to a large ventricular septal defect and varying degrees of right ventricular outflow tract narrowing [ 2 ]. The incidence of TOF is estimated at 5~7 per 10 000 live births and TOF accounts for 5~7% of all congenital heart lesions [ 3 ].…”
