The 360 Double Lasso Loop for Biceps Tenodesis: Tips and Tricks

Abstract: The management of the intra-articular portion of the long head of the biceps tendon (LHB) is a recurring topic in every discussion about shoulder pain. In massive rotator cuff tears or in tears of the superior third of the subscapularis tendon, our approach is to systematically perform a tenodesis of the LHB. In this Technical Note, we present our arthroscopic technique for LHB tenodesis at the articular margin of the humeral head using a single anchor and a 360 double lasso loop. This technique guaranties a s… Show more

“…Through the anterior portal, the LHBT was fixed intra-articularly about 2 cm distal to its origin. Minitape (Smith+Nephew) and Firstpass or Firstpass Mini (Smith+Nephew) were used with the double lasso-loop technique as described by Lafosse et al 18 , 19 …”

“…The working portals were: D-anterolateral, E-anterior (corresponding to the rotator interval) and B 0 -inferior posterolateral for the Hill-Sachs remplissage (Figures 2 and 3; Video S1). 14 The arthroscopic round-diagnostic identified a superior labral tear (SLAP), as well as an engaging Hill-Sachs lesion. The dynamic testing conducted during arthroscopy revealed the occurrence of anterior dislocation when passive external rotation was coupled with abduction of the arm.…”

“…A free suture tape (Arthrex, Naples, FL, USA) was inserted into the LHBT and a 360° pretzel like lasso was performed into and around the tendon, as demonstrated by Popescu et al . for dynamic anterior stabilization 9,14 . Suture ends were then retrieved through the E portal.…”
