The Accuracy of Serologic Diagnosis of Helicobacter pylori Infection in School‐aged Children of Mixed Ethnicity

Abstract: Sera and questionnaires were collected from 695/858 (81%) Swedish school children with mixed ethnic backgrounds within a cross-sectional, community-based study. Of 133 children with an ELISA OD value of > or = 0.1, all were screened with immunoblot and 107 made a 13C-UBT. The negative controls were 34/37 children from three school classes with an ELISA OD value of < 0.1 and volunteering for a 13C-UBT. An adjusted cut-off level for the ELISA of OD value 0.22 resulted in a sensitivity of 97.8%, a specificity of … Show more

“…To our knowledge, no previous study has systematically compared antibody levels in individuals from the same population with different manifestations of H. pylori infection. We have previously noted that when using ELISA for screening of Swedish school children at 10 years of age, our cutoff level previously established in culture-confirmed patients within a Swedish population would have resulted in a loss of sensitivity (15). We speculated that the need to lower the cutoff level might have been due to the fact that the children were 10 years of age, while the patients were adults.…”

“…It would also explain our previous observation that the cutoff level established for symptomatic patients needed to be lowered for screening purposes for 10-year-old children (15). The problem is of general relevance since cutoff levels in serological assays are established against a "gold standard", usually culture.…”

“…Information about family characteristics was obtained by a questionnaire completed by the parents. Blood was drawn to determine serologically the H. pylori infection status with an in-house enzyme-linked immunosorbent assay and immunoblot (Helico Blot 2.0, Genelabs Diagnostics, Singapore) (34). H. pylori-seropositive individuals were invited to undergo gastroscopy with culture of biopsy samples from the corpus and antrum.…”
