The analysis of circulatory system morbidity in adult population in the city of Voronezh

Relevance. Today, in preventive medicine, climatic conditions that have a pathological effect on the functional state of a person are increasingly being updated. the occurrence of exacerbations of many diseases can be causally associated with various weather conditions. Aim: to develop the main tasks for improving the organization of medical care for weather-dependent patients with diseases of the cardiovascular system. Material and methods. The assessment of personnel, material and technical support and the main performance indicators of an outpatient clinic was carried out on the example of the Voronezh city polyclinic No. 18 to develop the main tasks for improving the organization of medical care for weather-dependent patients with diseases of the cardiovascular system. Results. The main personnel problem is the low staffing of district therapists and specialists of a narrow service. One of the priorities for reducing the burden on medical hospitals is the organization of inpatient replacement medical care on the basis of outpatient clinics. The indicators for the implementation of state guarantees for the outpatient network for 2018, which were fully implemented, are given. The analysis of the planned load performance by polyclinic specialists is presented. Cardiological and neurological services carry out measures to reduce the risk of exacerbations of diseases with cerebral atherosclerosis, hypertension, and major neurological nosologies. Conclusion. Improving the organization of medical care for weather-dependent patients with cardiovascular diseases are: informing patients about the sources of specialized medical weather forecasts in the region, organizing the work of the medical prevention office, implementing an interdepartmental approach to providing health care to the most vulnerable groups of the population.

Relevance. Over the past decade and at the present time, diseases of the circulatory system continue to occupy a leading position. Despite the implementation of Federal and regional programs aimed at improving the level of cardiological care and reducing the mortality of adults and children, the assessment of the level and dynamics of diseases of the circulatory system remains an urgent task for the health system. Aim: to analyze the dynamics of the incidence of diseases of the circulatory system of adults and children in Voronezh. Material and methods. In order to allow territorial comparison of morbidity levels, relative indicators were calculated per 1000 population of the corresponding age (children, adults). The assessment of the level of morbidity was carried out by analyzing medical and statistical data on the population's access to medical care over a five-year period. Results. This study is devoted to the assessment of the incidence of diseases of the circulatory system, which was carried out by analyzing medical statistics on the population's access to medical care for five years. It was found that the average long-term level of cardiovascular diseases in the Voronezh city district is 24.79±1.40 cases per 1000 children under 14 years of age, and 403.14±21.36 cases per 1000 adults 18 years and older for the adult population. Over the past 5 years (2014-2018), there has been an increase in the incidence of both children and adults. Conclusion. Thus, knowledge of information about the morbidity of adults and children makes it possible to rationalize the resources of the urban health system to improve the availability and quality of medical care for people with cardiovascular diseases at the pre-hospital stage.

Currently, and over the past decade, intensive research is being conducted in the field of organizational, preventive and therapeutic work with weather-sensitive and weather-dependent patients suffering from cardiovascular pathology. One of the most discussed issues in practical medicine is the application of climate conditions, with a fixed frequency and time of exposure. Conditions of influence of climatic factors, in some cases, can exceed the limits of the norm and, accordingly, have a pathological effect on the functional state of a person. Thus, we used data from statistically reliable results of 928 questionnaires processed and analyzed, reflecting the medical and social characteristics of patients with cardiovascular pathology. Direct results were evaluated by analyzing changes in the dynamics of indicators. According to a questionnaire survey, 62% of patients who have chronic diseases and seek medical help for diseases of the cardiovascular system believe that weather factors have a significant impact on their health. of the patients who do not have chronic diseases, 38% gave positive answers to the question about the weather sensitivity of their health. As part of the pilot project to create a system of long-term care for elderly and disabled citizens in the Voronezh region, patients are informed about the sources of obtaining specialized medical weather forecasts in the region; interaction between the parties is organized to conduct sanitary and educational work; medical examinations are organized and conducted; training sessions are organized for relatives of citizens who have lost the ability to self-service. Further organizational, preventive and therapeutic work with weather-sensitive and weather-dependent patients in the Voronezh region is characterized by an increase in the quality of medical care for patients with cardiovascular diseases.
