The Analysis of Spontaneous Closure Mechanisms and Regeneration of Retinal Layers of a Full-Thickness Macular Hole

Abstract: The main mechanism leading to a spontaneous closure of FTMH is the "bridging" phenomenon. Vitreous detachment and vitreomacular traction release are not necessary conditions promoting the closure of FTMH. Shorter duration of symptoms and regeneration of photoreceptors IS/OS interface provide a better final BCVA.

“…Among these, the age of patients reported by most of the articles was greater than 60, except a 50-year-old case reported by Kelkar et al [9]. The average spontaneous closure time of IMHs was 99.5 ± 98.0 d. Morawski et al [7] suggested a 2- to 3-month observation time before surgery in posttraumatic eyes, since a quite number of cases showed spontaneous closure. In our review data, it seems that a 3-month observation time may also be considered in IMHs.…”

“…Although few publications have documented the closure of primary FTMH secondary to CME with topical steroids, 5 , 6 to our knowledge only one case of recurrent FTMH closure has been reported 4 . Clinical examination and SD-OCT confirmed the absence of traditional tractional forces such as epiretinal membrane or vitreomacular traction, as well as the bridging phenomenon, which is considered to be the leading mechanism process for spontaneous FTMH closure 7 . CME was speculated to be playing the main role in pathogenesis, reason why topical steroids were initiated.…”

“…This has been seen in the previous studies and signifies that retinal architecture and visual improvement may occur till late after macular hole surgery. [ 10 ]…”
