The association between carotid atherosclerosis and treatment with lithium and antipsychotics in patients with bipolar disorder

Abstract: Objectives: Patients with bipolar disorder are at high risk of cardiovascular diseases. Among cardiovascular diseases, coronary heart disease and stroke are the leading causes of premature death and both share the pathogenesis of arterial atherosclerosis. Increased carotid intima-media thickness is sensitive for detecting early atherosclerosis and a practical index for predicting cardiovascular diseases. However, few studies investigated carotid intima-media thickness in adults with bipolar disorder. We attemp… Show more

“…Since this was a naturalistic study and medications used to treat bipolar disorder were prescribed according to the primary care psychiatrist's judgment, patients who received lithium as the maintenance treatment in this study may take different kinds of psychotropic medications during the disease course of bipolar disorder. To estimate lithium exposure in relation to the duration of bipolar disorder, we calculated the ratio of lithium exposure in relation to bipolar illness chronicity (RDIC) using our previously described formula: total lifetime years of lithium treatment / total years since the onset of bipolar disorder (Tsai et al, 2020). The greater RDIC value indicated a more consistent treatment with lithium during the disease course of bipolar disorder.…”

“…For instance, clinical studies have revealed that lithium treatment reduces the risk of cardiovascular disease in patients with bipolar disorder. 26,34,35 In addition, laboratory studies have indicated that lithium administered at therapeutic levels ameliorates pathological hypertrophy and cardiac fibrogenesis in cellular and rat models of myocardial infarction. 6,36 Lithium likely achieves cardioprotective effects by inhibiting inflammatory responses, reactive oxidative stress, and endothelial dysfunction 37,38 and involves numerous signaling pathways.…”

“…AS has been recognized as a momentous pathological basis for cardiovascular diseases. The pathogenesis of AS is incredibly complex and has not yet been fully elucidated [ 1 , 3 ]. The biological behaviors of VSMCs exert important effects on the pathogenesis and development of AS.…”

“…With the acceleration of aging process in human society, the incidence rate of AS has been increasing year by year [ 2 ]. Besides, AS is the major contributor to death of cardiovascular disease worldwide, and also one of the main causes of acute cardiovascular disease [ 3 ]. This phenomenon has been widely concerned by the cardiovascular community.…”
