Objectives: The study aimed to evaluate different laboratory parameters in children and adolescent with type 1 diabetes mellitus (T1DM) with and without diabetic neuropathy. Methods: 60 children and adolescents with T1DM was enrolled in this study, they were classified into: 30 cases with T1DM from 2 to 5 years, 30 cases with T1DM for more than 5 years and 30 healthy children matched for age and sex as a control group. Blood samples were obtained from all cases to assess different laboratory investigation including hemoglobin (Hb) level, liver function test (ALT and AST), renal function test (urea and serum creatinine), fasting blood sugar (FBS), 2 h post prandial glucose (2hPP), lipid profile (serum cholesterol and triglycerides) and thyroid function tests. Results: There was significant value in serum creatinine, serum cholesterol, FBS and 2hPP glucose level between the studied groups. Conclusion: Patients with DM duration more than five years are associated with impaired renal function, hypercholesteremia and less glycemic control.

Introduction This study aimed to determine the relationship between HbA 1c variability and foot ulcer healing at 12 weeks and 12 months. Methods Using National Diabetic Foot Care Audit (NDFA) and hospital records, demographics, baseline ulcer characteristics and healing outcomes for subjects presenting with a foot ulcer between 2017–2022 were collected at 12 weeks and 12 months. Subjects had diabetes duration > 3 years and ≥ 3 HbA 1c recordings in the 5 years prior to presentation. Results At 12 weeks, factors associated with an active ulcer were presence on hind foot (adjusted odds ratios) (2.1 [95% CI 1.3–3.7]), ischaemia (2.1 [95% CI:1.4–3.2]), area > 1 cm 2 (2.7 [95% CI:1.7–4.2]) and diabetes duration > 24 years vs 3–10 (AOR 2.0 [95% CI 1.2–3.5]). After adjustment, HbA 1c variability 6–10 mmol/mol and > 14.5 mmol/mol had AOR of 1.76 (95% CI 1.1–2.8; p = 0.0192) and 1.5 (95% CI 0.9–2.6; p = 0.1148) of an active ulcer at 12 weeks vs variability < 6 mmol/mol. At 12 months, ischaemia (AOR 2.4 [95% CI 1.5–3.8]) and diabetes duration > 24 years vs 3–10 years (AOR 3.3 [95% CI 1.7–6.4] were significant factors. HbA 1c variability was not significant at 12 months. Conclusion In keeping with the national NDFA data, in our cohort ulcer characteristics, but not HbA 1c variability, were the key factors associated with ulcer healing at 12 weeks and 12 months.

Aims To investigate the impact of low socioeconomic status (SES) and/or membership in ethnic minority has on HbA1c before and during the COVID-19 lockdown. Methods A retrospective cohort study was conducted between March 2019 and March 2021, based on data from electronic medical records of 17,072 patients with type-2 diabetes, collected by Clalit (Israel’s largest health maintenance organization). Low SES was compared to high and ethnic minorities (Arabs and ultra-Orthodox Jews) were compared to the general Israeli population of mostly Jewish, but not ultra-Orthodox, Israeli citizens. Quantile regressions were used to examine the impact of SES and ethnic minority membership on HbA1c levels in the 0.10, 0.25, 0.50, 0.75, 0.90 quantiles before and during the lockdown. Results In the pre-lockdown period, patients with type-2 diabetes of low versus high SES, and Arabs versus the general population, had higher HbA1c. During the lockdown HbA1c levels of low versus high SES rose significantly in the 0.10 and 0.90 quantiles, and among Arabs HbA1c levels rose significantly across all quantiles, with a remarkable increment in the 0.90 quantile (from 0.316% in the pre-lockdown period to 0.730% in the lockdown period). Ultra-Orthodox Jewish diabetic patients had a marginally higher mean HbA1C level regardless of the period. Quantile regressions did not reveal a significant difference between the ultra-Orthodox Jewish and the general population. Conclusion The lockdown exacerbated disparities in glycemic control between low and high SES individuals and between Arab minority and the general population.
