Determinants of mental health professionals’ attitudes towards recovery: A review

Objective: The attitudes of mental health professionals towards consumers’ recovery are far more pessimistic than what is needed for the recovery-orientation to truly permeate systems of care. It has become pressing to depict determinants for these attitudes and how they evolve during professionalization. This, in the hopes to adjust not only medical education, but also ongoing training of professionals. Methods: A systematic search of PubMed and PsycINFO databases was conducted, yielding a net 15 303 records. Twenty-two publications from specific educational journals and reference lists were added. Finally, thirty-four full texts were read, from which twenty-two articles were included. Results: From the reviewed studies emerged five main determinants: profession, education, age, clinical experience, and nature of the contact with consumers. Traditional clinical placements during residency, negative experiences with acute patients, younger age and the professional attitudes of psychiatrists seem to all be determining factors for professionals’ pessimistic attitudes towards recovery. Conclusions: This review found specific determinants for attitudes in recovery and four out of five can be acted upon. For a recovery-orientation to be implemented across our mental health system, we formulate recommendations within the Canadian context for revision of curriculum, recovery-specific training, and operationalisation through state/provincial technical assistance centers.

From Adherence to Self-Determination: Evolution of a Treatment Paradigm for People With Serious Mental Illnesses

Treatment adherence and nonadherence is the current paradigm for understanding why people with serious mental illnesses have low rates of participation in many evidence-based practices. The authors propose the concept of self-determination as an evolution in this explanatory paradigm. A review of the research literature led them to the conclusion that notions of adherence are significantly limited, promoting a value-based perspective suggesting people who do not opt for prescribed treatments are somehow flawed or otherwise symptomatic. Consistent with a trend in public health and health psychology, ideas of decisions and behavior related to health and wellness are promoted. Self-determination frames these decisions as choices and is described herein via the evolution of ideas from resistance and compliance to collaboration and engagement. Developments in recovery and hope-based mental health systems have shepherded interest in self-determination. Two ways to promote self-determination are proffered: aiding the rational actor through approaches such as shared decision making and addressing environmental forces that are barriers to choice. Although significant progress has been made toward self-determination, important hurdles remain.

Despite increased interest in the concept of recovery, not enough is known about the challenges of implementing recovery models in mental health care settings. Findings are presented from a 3-year feminist ethnographic study that followed recently deinstitutionalized women and men as they moved into psychiatric tertiary care facilities in British Columbia where a psychosocial rehabilitation model based on recovery principles was implemented. We found that inconsistent staff training and stretched community supports have resulted in uneven implementation that does not yet maximize opportunities for people's recovery. Further, care is organized and delivered in ways that emphasize individual needs as opposed to social and collective needs based on factors such as gender, ethnicity, and culture. These findings indicate that greater political will, as measured in commitments to community-based mental health services, is required to fully realize the philosophy of recovery and equitable mental health care.
