The Bilobed Racquet Flap or Extended Seagull Flap for Thumb Reconstruction: A Case Report

Abstract: The treatment of extensive soft tissue defects in the thumb with dorsal metacarpal artery flaps has been previously reported in the literature. Island flaps from the dorsum of the index and long fingers have been the subject of many reports and studies. However, when the defect involves the whole thumb, a 360° circumferential defect, standard first or second dorsal metacarpal artery flaps are usually insufficient. There are fewer reports on the use of bilobed flaps for this application and we have found no rep… Show more

“…Tissiani et al describe the versatility of the bilobed flap in a case series of 42 patients who had bilobed flaps done across a wide range of anatomical sites including cheek, upper lip, zygoma, upper limb, and lower limb. [11][12][13] Other options for the repair of defects in Zone II include the nasolabial flap, dorsal nasal flap, forehead flap, and full-thickness skin graft which are time-tested and reliable but complications like swelling, scarring, flap necrosis, infection, and bleeding are well known. Due to the crescentic shape of the flap, it is at risk of developing pincushion deformity as a result of tissue contraction beneath the flap.…”

“…28 Recently, Couceiro et al extended the flap by adding a lobe supported on the second dorsal metacarpal artery, producing a bilobed flap, which is used to close very large defects of the thumb. 29 The Holevich flap is a modification of the Hilgenfeldt flap that utilizes the same FDMA as both Foucher and Hilgenfeldt, but with an attached racquet of skin extending from the dorsal hand to the proximal index finger. “…It can be executed either as a racquet or as an island flap. 9 10 The paths of the first and second metacarpal artery are identified with a Doppler's probe and marked with a needle. The dissection is once again started ulnarly, the area on the dorsum of the middle finger and a small section of skin connecting the index and middle finger are included as well.…”

“…Couceiro et al 10 described the use of an extended seagull flap for the coverage of a thumb degloving injury, they included a very big skin paddle and the skin on the dorsum of the proximal phalanxes of the index and middle finger.…”
