The black hole of the transition process: dropout of care before transition age in adolescents

Abstract: Recent evidence confirms the risks of discontinuity of care when young people make a transition from child and adolescent mental health services (CAMHS) to adult mental health services (AMHS), although robust data are still sparse. We aimed to identify when and how patients get lost to care during transition by tracking care pathways and identifying factors which influence dropping out of care during transition. This is a retrospective observational study of 760 patients who reached the transition age boundary… Show more

“…Many people experience discontinuity of care and drop out during this critical period (Reneses et al, 2023). Inadequate transition processes not only pose risks to mental health, but can also exacerbate the likelihood of engaging in at-risk behaviors.…”

“…However, even if research and innovative practices in transitional care for young people with chronic conditions have improved in the last few years, barriers remain. Many people experience discontinuity of care and drop out during this critical period (Reneses et al ., 2023). Inadequate transition processes not only pose risks to mental health, but can also exacerbate the likelihood of engaging in at-risk behaviors.…”

“…Due to the fragmented nature of care services, children and adolescents with enduring and interrelated psychiatric- and social care needs experience barriers to accessing and engaging in regular office-based mental healthcare services (Anderson et al, 2017 ; Markoulakis et al, 2023 ). This can lead to inefficient care processes with care discontinuity and even care avoidance (Borah et al, 2021 ; Reneses et al, 2022 ; Skehan & Davis, 2017 ). Multidisciplinary Youth Flexible Assertive Community Treatment (Flexible ACT) teams address the multifaceted needs of these young people (0–24 years of age) in an integrated manner.…”

“…Up to 56% of young people with ongoing mental health issues experience cessation of care in the 12 months leading up to transition age. Risk of dropout decreases for more serious mental illness including psychosis, bipolar disorder, and eating disorders (Reneses et al., 2022). A review of transition to AMHS (Reale & Bonati, 2015) highlighted issues that needed to be considered in order to reduce this lack of continuity, including: viewing transition as a process rather than as something that happens strictly at a fixed age; allowing parents greater involvement in their child's care across the transition process; addressing the cultural divide between CAMHS and AMHS; further evaluation of transition that informs the reasons why dropout occurs and the implications for improving models of care.…”
