In 1987, a monoclonal antibody, RP215, was generated against the OC-3-VGH cancer cell line and shown to react with a carbohydrate-associated epitope located mainly on the heavy chains of immunoglobulins, designated as CA215, which are expressed on the surface of almost all cancer cells and not on normal immune cells. CA215 and cancerous immunoglobulins were affinity-isolated from the shed culture media of the same ovarian cancer cell line by using RP215-linked and anti-human immunoglobulin G-linked affinity chromatography, respectively. They were then immobilized separately as general ligands to isolate any protein components from pooled human sera which demonstrated affinity to CA215 and/or cancerous immunoglobulin G. The affinity-isolated components were then subjected to molecular analysis by liquid chromatography-tandem mass spectrometry (LC-MS/MS). Among the detected protein components, more than 72% were found to be commonly recognized by both CA215 and cancerous immunoglobulins. Some have previously been identified as endogenous proteins or fragments in human serum. They are considered generally, as cancer biomarkers, according to their pro-cancer (e.g. C4b binding protein α chain, complement C3, complement factor H, serotransferrin, and vironectin), anti-cancer (e.g. 35 kDa inter-α-trypsin inhibition heavy chain 4, anastellin, apolipoprotein A-1, fibrinogen β chain, and, keratin type I cytoskeletal 9), or autoimmune (specific immunoglobulin G) properties, respectively. Therefore, besides serving a role for the growth/proliferation of cancer cells, cancerous immunoglobulins can also interact directly or indirectly with many other serum protein components or fragments for unspecified reasons and unknown mechanisms of action.

At the time of diagnosis, 20% of patients with colorectal cancer present metastasis. Among individuals with primary lesions, 50% of them will develop distant tumours with time. Therefore, early diagnosis and prediction of aggressiveness is crucial for therapy design and disease prognosis. Tumoral cells must undergo significant changes in energy metabolism to meet increased structural and energetic demands for cell proliferation, and metabolic alterations are considered to be a hallmark of cancer. Here, we present the ATP‐binding cassette transporter (ABCA1), a regulator of cholesterol transport, as a new marker for invasion and colorectal cancer survival. ABCA1 is significantly overexpressed in patients at advanced stages of colorectal cancer, and its overexpression confers proliferative advantages together with caveolin‐1 dependent‐increased migratory and invasive capacities. Thus, intracellular cholesterol imbalances mediated by ABCA1 overexpression may contribute to primary tumour growth and dissemination to distant locations. Furthermore, we demonstrate here that increased levels of apolipoprotein A1 (APOA1), a protein involved in cholesterol efflux and high‐density lipoprotein constitution, in the extracellular compartment modulates expression of ABCA1 by regulating COX‐2, and compensate for ABCA1‐dependent excessive export of cholesterol. APOA1 emerges as a new therapeutic option to inhibit the promotion of colorectal cancer to metastasis by modulating intracellular cholesterol metabolism. Furthermore, we propose apabetalone, an orally available small molecule that is currently being evaluated in clinical trials for the treatment of atherosclerosis, as a new putative therapeutic option to prevent colorectal cancer progression by increasing APOA1 expression and regulating reverse transport of cholesterol.

In the slipstream of targeting the adaptive immune system, innate immunotherapy strategies are being developed. In this context, technologies based on natural carrier vehicles that inherently interact with the innate immune system, are increasingly being considered. Immunoregulatory nanotherapeutics based on natural apolipoprotein A1 (apoA1) are discussed here. This protein is a helical, amphipathic macromolecule and the main constituent of high-density lipoprotein. In that capacity, apoA1 interacts specifically with innate immune cells, such as monocytes and macrophages, to collect and transport lipophilic molecules throughout the body. Exactly these unique features make apoA1 a compelling elementary constituent of biocompatible self-assembled nanotherapeutics. Such apoA1-based nanotherapeutics (A1-nanotherapeutics) can be engineered and functionalized to induce or mitigate an innate immune response or to orchestrate an adaptive immune response through antigen delivery to dendritic cells. The authors first discuss apoA1's properties and how these can be exploited to generate libraries of A1-nanotherapeutics using advanced manufacturing approaches such as microfluidics or continuous flow methods. Using high-throughput in vitro screening methods and in vivo imaging to identify promising formulations are then recommend. Finally, three distinct immunotherapy strategies are proposed to effectively treat a variety of diseases-including cancer, infection, and cardiovascular disease-and promote allograft survival in transplantation.
