The Circadian System Contributes to Apnea Lengthening across the Night in Obstructive Sleep Apnea

Abstract: The endogenous circadian system plays an important role in the prolongation of respiratory events across the night, and might provide a novel therapeutic target for modulating sleep apnea.

“…3 After a normal night of sleep and baseline testing, participants underwent 10 recurring 5-hours 20-min of ‘behavioural cycles’ of 2-hours 40-min of sleep opportunities and 2-hours 40-min of standardised waking episodes 3. Approximately 1 hour after each sleep episode, participants performed mild intensity cycle ergometer exercise for 15 min at 50% predicted maximal heart rate (Karvonen’s formula4).…”

“…Interestingly, the MT1 receptor mediates vasoconstriction, while the MT2 receptor mediates vasodilation [32]. By ameliorating the endogenous circadian system, melatonin may provide a novel therapeutic target for modulating sleep apnea-induced cardiovascular disease and hypertension [33]. Furthermore, melatonin shows good compliance and moderate efficiency for the treatment of SBDs.…”

“…Although there is limited data on the role of the circadian clock in OSA (60), it may play a significant role in this disease (61)(62)(63). We found dysregulation of circadian gene expression in multiple cell types.…”
