Purpose The purpose of this study was to develop and evaluate the psychometric properties of (a) a glaucoma medication selfefficacy scale and (b) a glaucoma outcome expectations scale.Patients and Methods Two instruments were developed: a glaucoma medication selfefficacy scale and a glaucoma outcome expectations scale. Packets containing (a) the instruments and patient demographic questions and (b) a letter explaining the study were distributed to 225 glaucoma patients from three ophthalmology practices between August and December 2007. The instrument was completed by 191 patients for a response rate of 85%. Principal components factor analysis with a varimax rotation and Cronbach's a reliability were used to analyse the data. To assess discriminant validity, we administered the scales and two self-reported measures of adherence in a separate sample of 43 glaucoma patients who were currently using at least one glaucoma medication.Results Our results yielded a 21-item self-efficacy in overcoming barriers that might interfere with the use of glaucoma medications scale, a 14-item self-efficacy in carrying out specific tasks required to use eye drops correctly scale, and a four-item glaucoma outcome expectations scale. Results of the Cronbach's a reliability indicated that the scales are internally consistent. The selfefficacy scales were both significantly associated with two patient self-reported measures of glaucoma medication adherence, which show discriminant validity.Conclusions Eye care providers and researchers can use these scales to identify patients with low self-efficacy in using their glaucoma medications and patients who do not believe that following their eye care providers' advice can help their vision.

The aim of this review was to summarize literature in view of patient-reported outcome (PRO) instruments for glaucoma and provide guidance on how outcomes are best assessed based on evidence about their content and validity. A systematic literature review was performed on papers describing the developmental process and/or psychometric properties of glaucoma or vision-specific PRO-instruments. Each of them was assessed on their adherence to a framework of quality criteria. Fifty-three articles were identified addressing 27 PRO-instruments. In all, 18 PRO's were developed for glaucoma and 9 for diverse ophthalmologic conditions. Seven instruments addressed functional status, 11 instruments quality of life and 9 instruments disease and treatment-related factors. Most of the instruments demonstrated only partially adherence to predefined quality standards. The tools for assessing functional status were of poor quality, while the Glaucoma Quality of Life Questionnaire and the Vision Quality of Life Index were well-developed QoL measures, yet only validated using classical techniques. The Rasch-scaled QoL-tools, IVI and VCM1 need to improve their item-content for glaucoma patients. The questionnaires to measure adherence should improve their validity and the Treatment Satisfaction Survey for Intra Ocular Pressure pops out as the highest quality tool for measuring topical treatment side effects. This review revealed that most PRO-instruments demonstrated poor developmental quality, more specifically a lack of conceptual framework and item generation strategies not involving the patients' perspective. Psychometric characteristics were mostly tested using classical validation techniques.

ÖzetAmaç: Glokom olgular›n›n tedaviye uyum oranlar›n›n incelenmesi ve glokomda hasta uyumunu etkileyebilecek faktörlerin ortaya konulmas›. Gereç ve Yöntem: Tan›mlay›c› ve kesitsel bu çal›flmaya dahil edilen 186 olguya, demografik ve klinik özelliklerini ortaya koymak amac›yla bir anket uyguland›. Ard›ndan olgular tedaviye hasta uyumunu de¤erlendirmek üzere oluflturulan 3 kriter yönünden incelendi. Bunlar; son 1 ayda 5 den daha az veya çok kez ilac›n aksat›lmas›; tedavinin do¤ru veya yanl›fl bilinmesi ve göze ilaç damlatma konusunda olgunun baflar›l› veya baflar›s›z olmas›yd›. Hastaya ait demografik ve klinik özelliklerin bu kriter gruplar› içerisindeki da¤›l›m›na bak›larak, tedaviye hasta uyumunda bozulmaya sebep olabilecek faktörler belirlenmeye çal›fl›ld›. Sonuçlar: Olgular›m›z›n 54'ü (%29,9) son 1 ay içinde 5'ten daha fazla kez ilac›n› aksatt›¤›n› ifade etti. Yetmifl befl (%40,3) olgu tedavisinin detaylar›n› yanl›fl bilmekteydi ve 45 olgu (%24,2) damlay› göz içine do¤ru bir teknikle damlatmay› baflaramad›. Tedavisini s›k aksatan olgularda unutkanl›k, glokom hakk›nda yetersiz bilgiye sahip olma, düflük çukurluk/disk oran›, fazla say›da antiglokomatöz ilaç kullan›m› ve negatif aile öyküsü daha fazla tespit edildi. Tedavi protokolünü yeterince bilmeyen olgular›n glokom hakk›nda bilgi düzeyi düflük, kulland›¤› antiglokomatöz say›s› fazla, glokom süresi k›sa ve e¤itim seviyesi düflük bulundu. Göz içine ilaç damlatmada baflar›s›zl›¤›n ise yafll›, üst ekstremitesinde k›s›tl›l›¤› olan ve glokom süresi k›sa olgularda daha s›k oldu¤u görüldü. Tart›flma: Glokom hastalar›ndaki tedaviye uyumsuzlu¤a sistematik bir flekilde yaklafl›lmal›d›r. Glokom tan›s› konulan veya etkin bir tedaviye ra¤men yeterli G‹B düflüflü sa¤lanamayan olgularda, tedaviye hasta uyumunu bozabilecek faktörler de¤erlendirilmelidir.
