The Clinical Value of Salivary Aldosterone in Diagnosis and Follow-Up of Primary Aldosteronism

Abstract: Primary aldosteronism (PA), the most common form of secondary hypertension, causes relevant morbidity. The value of salivary measurements of aldosterone in clinical routine in PA so far has not been assessed. First, we analyzed salivary and plasma aldosterone concentrations of 42 patients with PA and 37 hypertensive controls (HC) during a sodium infusion test prospectively. Second, morning salivary and plasma aldosterone concentrations as well as diurnal saliva aldosterone profiles were analyzed in 115 patient… Show more

“…Measurement and interpretation of the ARR is challenging due to various factors that impact on aldosterone and renin levels [1,2,[7][8][9][10][11]. A key issue is that several, in particular antihypertensive, drugs interfere with the renin angiotensin aldosterone system [1,2,8,12,13].…”

“…3). However, ACTH action still provides aldosterone precursors and increases aldosterone secretion, explaining its maintained circadian rhythm [105][106][107] and higher aldosterone values in AVS studies with vs. without ACTH infusion [95,103,104].…”

“…Primary aldosteronism (PA) represents the most frequent curable form of arterial hypertension [1][2][3]. Its relevant cardiovascular comorbidities, with potential reversal or improvement after early di-agnosis and therapy, justify a screening in high-risk populations [4][5][6][7][8][9][10]. These include patients with resistant hypertension, spontaneous or diuretic-induced hypokalemia, adrenal incidentaloma, or sleep apnea [11].…”
