Objective: The aim of this study was to explore beliefs and behaviors of opioid pain medications among patients undergoing elective surgery. Background: Opioid dependence after surgery is a major contributor to the ongoing opioid epidemic. Recent efforts by surgeons and health systems have sought to improve the education patients receive regarding safe opioid use after surgery; however, little is known about patients’ pre-existing beliefs surrounding opioids. Methods: Semistructured interviews were conducted with patients who underwent 1 of 4 common elective surgical procedures at 1 institution. Patients were specifically asked about their knowledge and beliefs about opioids before surgery and their opinions of opioid-sparing recovery after surgery. Coding was conducted through iterative steps, beginning with an initial cycle of rapid analysis, followed by focused coding, and thematic analysis. Results: Twenty-one patients were interviewed. Three major themes emerged regarding patient opinions about using opioids after surgery. First, there was widespread awareness among patients about opioid medications, and preoperatively, patients had specific intentions about using opioids, often informed by this awareness. Second, patients described a spectrum of opioid related behavior which both aligned and conflicted with preoperative intentions. Third, there was tension among patients about opioid-free postoperative recovery, with patients expressing support, opposition, and emphasis on tailoring recovery to patient needs. Conclusions: Patients undergoing common surgical procedures often arrive at their surgical encounter with strong, pre-formed opinions about opioids. Eliciting these preexisting opinions may help surgeons better counsel patients about safe opioid use after surgery.

In this case study, I draw attention to key Nevada Latina/o legislators’ policy ways of knowing and their higher education policy priorities. A focus on the policy actors uncovered structural, racial, and cultural assumptions in policy-making often absent in the exclusive analysis of policy interventions. Their policy ways of knowing were shaped in at least three ways: acknowledging and naming the sources of structural inequities, embracing political humility, and challenging policy knowledge-generating practices.

Although Model-Based Systems Engineering (MBSE) is quickly becoming adopted in Systems Engineering (SE), there have not been many studies evaluating literature to determine the perceived value of implementing MBSE. This research first identifies and discusses previous studies on the justification or rejection of MBSE. This study investigates how the SE community perceives the value of MBSE by coding and analyzing positive and negative descriptions of MBSE; perceived benefits and drawbacks of implementing MBSE; and the evidence and metrics used to substantiate and measure each claim about MBSE. From 60 academic sources, this study collected and coded over 2900 claims on MBSE. Our findings determine the most positive attributes of MBSE to be Verification & Validation Capability, Consistency, Reasoning, and Risk & Error Manageability, while the most negative attributes were Approach Understandability, Acceptability, Familiarity, and Approach Complexity. The most-stated benefits were Reduced Time, Better Communication/Information Sharing, Reduced Costs, and Better Analysis Capability. The most claimed drawbacks were Increased Costs, Increased Time, Increased Effort, and Worsened Capability. A large share of claims (47%) about MBSE was based on author opinions. Most claims (86%) were not substantiated by a metric.
