The coexistence of Behçet's disease and ankylosing spondylitis

Abstract: We present the case of a young woman suffering from both ankylosing spondylitis and Behçet's disease, associated with a severe inflammatory arthritis. Although the HLA phenotype was positive for HLA-B27 and negative for HLA-B5, the clinical findings, especially the joint manifestations of Behçet's disease, seem to appear dominantly. The meaning of this rare combination was discussed with a review of the literature.

“…In addition, several patients with coexisting AS and BD have been described in the literature (11,(16)(17)(18)(19). However, other authors have found that the prevalence of AS or SI in BD has not been different from that in the general population (20)(21)(22)(23)(24).…”

“…Dubost et al, [9] reported tree cases of BD and AS coexistence among 11 patients with BD. Borman et al, detected HLA-B27 positivity and HLA-B51 negativity in a 29 years-old female patient with co-existing AS and BD [7]. Similarly, Etaouil et al, found HLA-B27 positivity and HLA-B51 negativity in 2 cases with combined AS and BD [10].…”

“…Joint manifestations are 51%, artheralgia 35%, arthiritis 23% and AS 2% [5]. Knee, ankle, and wrist involvement is very frequent, while distal interphalangeal joint, vertebra and sacroiliac involvement is very rare [7]. AS is a chronic inflammatory disease in which vertebra and sacroiliac joint is involved [6,8].…”

“…Although BD was reported to have a strong association with HLA-B51 rather than with B27, it was concluded that it is not a subgroup of the seronegative SpA (2). On the other hand, the number of reported cases of coexisting BD and ankylosing spondylitis (AS) has increased (3)(4)(5)(6)(7)(8)(9). Recent reports showed that proinflammatory cytokines such as tumor necrosis factor (TNF) -alpha and genetic factors play important roles in the pathogenesis of both diseases (10)(11)(12)(13).…”
