The Collaborative Chronic Care Model for Mental Health Conditions

Abstract: Background: Extensive evidence indicates that Collaborative Chronic Care Models (CCMs) improve outcome in chronic medical conditions and depression treated in primary care. “…Between 2016 and 2018, as part of a partnered evaluation with the VA’s Office of Mental Health and Suicide Prevention (OMHSP), our team conducted a hybrid type II stepped wedge cluster randomized trial to implement the evidence-based CCM in BHIP teams.
