The Colorectal Tumor Microenvironment: The Next Decade

Abstract: Colorectal cancer cells establish a crosstalk with the tumor microenvironment, such that implantation and development of the tumor is generally favoured. CRC progression depends on mutations in the tumor's oncogenic pathways as well as metastasis suppressor genes, but is also influenced by the inflammatory components in the microenvironment. Inflammation results from the dietary intakes and is either compounded or counterbalanced by our lifestyles. Whether driven by intrinsic pathways or infection, inflammatio… Show more

“…Advanced age, immunosuppressive cytokines from chronic inflammation, tumor-derived immunosuppressive factors and surrendered immune cells, such as MDSCs constitute CRC microenvironment [ 34 , 35 ]. In TCM, the key pathogenesis of CRC immunosuppressive microenvironment is “Spleen-deficiency” [ 36 ].…”

“…Vice versa, in response, the microenviroment tries to encompass such growth by creating an inflammatory milieu, that intends to hamper tumor progression. In such a setting of altered immune response, posed by so many intersecting signals, the released inflammatory mediators in reality end up by perpetuating rather than co-localizing the cancerous transformation, thus permitting progression and allowing invasion [7,8,68].…”

“…There is some evidence that cancer cells establish a profitable interplay with bystander cells of their microenvironment, to receive help for growth and metastasis, nutrition and protection from immune recognition . The cross talk between tumor and stroma occurs through the modulation/release of nucleic acid messengers (DNA, mRNA and microRNAs [miRNAs]) and soluble factors acting locally and at a distance to modify the cells' behavior.…”

These modulators are ultimately detectable in peripheral blood, where they can serve as biological markers of cell transformation useful for early diagnosis and for predictive and prognostic purposes for developing minimally invasive lowburden screening tests.…”
