The Effect of Acupressure on Pain in Cancer Patients With Bone Metastasis: A Nonrandomized Controlled Trial

Background: Pain is a serious and common problem in bone metastases. For this purpose, complementary and supportive practices are also applied along with medical treatment. This study was conducted for the purpose of evaluating the effect of acupressure on pain in cancer patients with bone metastasis. Methods: The study was conducted in a nonrandomized controlled trial with patients who applied to the radiotherapy unit of an oncology hospital. The data of the study were collected by using a questionnaire and the Visual Analog Scale. A total of 8 acupressure sessions, which lasted for approximately 10 minutes each (with warming and acupressure periods), was applied to the intervention group. The data were analyzed by using χ2 test, paired t test, and Pearson’s correlation coefficient. Results: It was determined that the pain mean score of the intervention group was 7.6 ± 1.9 before the acupressure and decreased to 6.8 ± 1.9 after the acupressure and this result was statistically significant. On the other hand, no significant difference was determined in the pain mean score of the control group. Conclusions: Acupressure is applicable for cancer patients with bone metastasis by nursing staff after receiving brief training and may make a difference in relieving pain of the patients. Further well-designed trials should be conducted.

Summary Objectives: In this study, we aimed to investigate the effects of using transdermal fentanyl (TDF) on cognitive functions in cancer pain. Methods: After approval and informed consent of patients by the Ethical ÖzetAmaç: Bu çalışmada, kanser ağrısında transdermal fentanil (TDF) kullanımının kognitif fonksiyonlar üzerine olan etkisinin incelenmesi amaçlandı. Gereç ve Yöntem: Etik kurul ve hasta onamı alınarak daha öncesinde opioid tedavisi almamış 50 kanser ağrılı hasta çalışma kapsamına alındı. Ağrı değerlendirmesi Vizüel Analog Skala (Visual Analogue Scale-VAS), kognitif fonksiyon muayenesi Addenbrook Kognitif Muayenesi gözden geçirilmiş versiyonu (Addenbrooke' s Cognitive Examination final revised version -ACE-R) ve günlük aktivite düzeyi Doğu Onkoloji Grubu Performans Ölçütü (Eastern Cooperative Oncology Group Performance Status -ECOG) ile değerlendirildi, yan etkiler kaydedildi. Hastaların ilk başvurularında algolojik değerlendirmeleri yapıldı ve ACE-R uygulanarak normal kognitif fonksiyon düzeyleri tespit edildi. Olgularda 25 μg/saat TDF ile tedaviye baş-landı, VAS ≤ 2 olacak şekilde tedavilerinin belirli aşamalarında doz artışı yapıldı, 30. günde yeterli analjezik etkinlik sağlan-dıktan sonra ACE-R yeniden uygulandı. Tüm hastalar kendi aralarında, TDF tedavisi öncesi ve sonrası olmak üzere ACE-R toplam skoru ve alt grupları (dikkat-oryantasyon, bellek, akıcılık, dil, görsel-mekansal yetenekler) açısından karşılaştırıldı. Bulgular: Çalışma sonunda tedavi sonrası, öncesi ile karşılaştırıldığında dikkat, oryantasyon, bellek, akıcılık, dil ve ACE-R toplam skorlarında istatistiksel olarak anlamlı artış saptandı. Görsel ve mekansal yeteneklerde ise anlamlı bir değişme görüle-medi. Olguların günlük aktivite düzeyi başlangıca göre değişmedi. Sonuç: Kanser ağrısında TDF kullanımının kognitif fonksiyonlarda bozulma yapmadığı görüldü, hatta bazı kognitif test skorlarında anlamlı düzelme olduğu saptandı. Ağrının olmaması da hastaların yaşam kalitesini artırdı.Anahtar sözcükler: Kanser ağrısı; kognitif fonksiyonlar; transdermal fentanil.

Abstract: The TDF patch is a good choice for cancer pain treatment for both adult and geriatric patients. Since it was observed that the incidence of side effects was higher in the geriatric patients, they should be treated more carefully.
