The Conducting Tissues in Congenitally Corrected Transposition

Abstract: posterior septum ascended toward the posterior extent of the interatrial septum and usually joined it posterior to the coronary sinus (fig. lb). The gap between the two septa was filled by that segment of the membranous septum which intervened between the morphological left atrium (left-sided) and the morphological left ventricle (right-sided). (fig. lc and d). When traced anteriorly, the interatrial and in-Circulation, Volume 50, November 1.974 916 at University of Hawaii--Manoa on June 16, 2015 http://circ.a… Show more

“…6, 7 Histologic investigation has disclosed that fibrosis and disruption in an elongated non-branching His bundle, together with fibrosis, occurs at the junction with the anterior node in situs solitus with L-loop CCTGA (without pulmonary atresia). [8][9][10] Electrophysiologic observations are consistent with the histological studies of CCTGA. 11…”

“…Considering that the conduction tissue lies on the left side of the septum in CCTGA,29, 30 incidence of complete heart block might be the consequence of mitral valve surgery or ventricular septal defect closure, instead of TVR. In the 3 cases of surgically induced complete heart block, 2 underwent mitral valve surgery and the other 1 had ventricular septal defect closure.…”

“…52,53 The second anomalous AV node and bundle are usually anterior, and the long penetrating bundle is vulnerable to fibrosis with advancing age. This makes the conduction system somewhat tenuous, with a progressive incidence of complete AV block occurring at Ϸ2% per year.…”
