This study examined therapists' emotional and cognitive responses to patients with borderline personality disorder (BPD) versus patients with major depressive disorder (MDD). Therapists' narratives (N=80) were elicited using the Relationship Anecdotes Paradigm interview method and then scored according to the core conflictual relationship theme-Leipzig/Ulm method (CCRT-LU; Albani et al., 2002). The emotional valences of therapists' responses were significantly more negative toward patients with BPD. Therapists differentially experienced patients with BPD as typically withdrawing and patients with MDD as attending within sessions. Therapists felt less satisfied in their therapeutic role with BPD despite a consistent wish to help patients. Findings support the utility of the CCRT-LU method in investigating therapist relational experiences and underscore the challenges for BPD treatment.

Using Structural Analysis of Social Behaviour (SASB), the communicative means by which a patient succeeds in re-enacting his or her own maladaptive relationship patterns in the interaction with the therapist is examined. This line of investigation stems from the work of Henry and co-workers, who have shown that therapists often unknowingly allow themselves to be drawn into subtly hostile interactions with patients, and that such interactions are associated with a negative outcome. In a mixed quantitative-qualitative single case study, the relationship narratives of a female patient from two therapy sessions are analysed using a cluster-analytic procedure in order to identify her repetitive relationship patterns. Then, using a short sequence of interaction, it is demonstrated how a central maladaptive pattern is re-enacted in the therapeutic interaction and gradually takes hold. These results are interpreted as evidence that patient communication contains paradoxical demands which can lead therapists to become part of the maladaptive pattern of interaction.

This naturalistic study focuses on changes in central relationship patterns after long-term dynamic psychotherapy (mean duration = 3 years) with experienced psychotherapists. Relationship patterns before and after treatment were assessed according to the core conflictual relationship theme (CCRT) method, which includes 3 components: most frequent wish, response from others, and response of the self. Overall, the main CCRT did not change substantially after therapy. There were, however, a few significant changes for some of the standard cluster categories after therapy. Patients' flexibility regarding their use of different wishes and negative responses from others and the self increased, as did the frequency of positive responses from others and the self. The positive changes in relationship patterns were only rarely related to changes in symptoms and character pathology.
