The Costs and Consequences of Adequately Managed Chronic Non‐Cancer Pain and Chronic Neuropathic Pain

Abstract: Strenuous efforts should be put into obtaining good levels of pain relief for people in chronic pain, including the opportunity for multiple drug switching, using reliable, validated, and relatively easily applied patient-centered outcomes. Detailed, thoughtful and informed decision analytic policy modeling would help understand the key elements in organizational change or service reengineering to plan the optimum pain management strategy to maximize pain relief and its stream of benefits against budgetary and… Show more

“…While the mechanisms underlying allodynia are still partially unknown, neuropathic hyperalgesia is associated with up-regulation of molecules, such as substance P and the excitatory neurotransmitter glutamate, which enhance the transmission of painful stimuli in DRG and spinal dorsal horn neurons (Fujita et al, 2007;Marchand et al, 2005;Ueda, 2006). Based on a review of studies published since 2000, it is estimated that 7% of the world's population is affected by NP (Andrew et al, 2014). However, an epidemiological review of chronic pain found that, within a given population, there are still no accurate estimates available for the prevalence of NP (Smith and Torrance, 2012).…”

“…The emotional toll exacted by chronic pain is well reported, with multiple studies noting the association between chronic pain and depression, anxiety, and loss of one's social role. 1,5,6,22 PCPs frequently expressed frustration and concern in their questions to specialists, a finding that is likewise reflected in the literature. 23,24 One study found that nearly three quarters of PCPs considered providing chronic pain care to be a major source of frustration.…”

“…The results of a systematic review showed that pain interact with activities of daily living, work and work efficiency, and reduced quality and quantity of life in cancer patients. One the other hand, this review showed that effective pain therapy resulted in improvements in fatigue, sleep, depression, quality of life, and work in these patients (Andrew et al, 2014).…”
