The critical care clinical pharmacist: Evolution of an essential team member

Abstract: Critical care pharmacists are recognized as essential members of the critical care team as a result of contributions to medication safety, improved patient outcomes, and reduced drug costs and as a source of drug information and provider of education. A growing number of pharmacists practice in critical care. Additional opportunities exist and can be met if an adequate supply of trained specialists can be developed.

“…23 Although clinical pharmacists are not specifically listed, several reports have described and documented the impact of critical care pharmacists within a multidisciplinary ICU team. [24][25][26][27] Evaluations of clinical pharmacists in the ICU setting have demonstrated the potential for improved patient outcomes and cost savings in critically ill patients, including those with VAP. [25][26][27] One of the greatest barriers to implementing a prevention program is gaining administrative support and funding.…”

“…infusion period can be used to increase the T > MIC and lower the peak concentration. 26,27 The infusion period can be extended through either (a) continuous infusion (i.e., administering a loading dose and then a pump to administer the total daily intravenous dose over a 24-hour period), or (b) prolonged infusion (i.e., administering the same dose and dosing interval but increasing the duration of infusion, such as from 30 minutes to 3 hours).…”

“…[52][53][54][55][56][71][72][73] Clinical pharmacists trained in pediatrics should be integrated into inpatient rounds and used for education of staff and families in all settings as often as possible. 42,[73][74][75] The AAP has launched webinars and Web sites and has partnered with other national leaders to offer specific tools, resources, and links to best health care safety practices for children 44,45,76-78 (Appendix 4). Collaborative implementation and measurement of both the process (adherence to practice) and clinical outcomes of shared strategies are necessary to track and refine care practices for all children.…”

“…Several publications have detailed the beneficial role of pharmacists in critical care settings. [2][3][4] The American College of Clinical Pharmacy (ACCP) Critical Care Practice and Research Network (PRN) described the difficulty of standardizing the training that should be required for a specialty degree/certification in critical care pharmacy practice. 5 Flexibility within pharmacy curriculum design was strongly encouraged to allow for incorporation of critical care topic discussion and learning.…”
