The Cultural Politics of ‘Implementation Science’

Abstract: Despite the growing profile of 'implementation science', its status as a field of study remains ambiguous. Implementation science originates in the evidence-based movement and attempts to broaden the scope of evidence-based medicine to improve 'clinical effectiveness' and close the 'implementation gap'. To achieve this agenda, implementation science draws on methodologies from the social sciences to emphasise coherence between qualitative and quantitative approaches. In so doing, we ask if this is at the expen… Show more

“…As we continue to define our implementation constructs and outcomes, and better understand the theories and conceptual approaches, we should incorporate the testing of the boundaries of our implementation science frameworks in LMICs, as the majority of the frameworks and measures were developed in HICs. Perhaps now it is time for us to consider how is implementation being conceptualized (19). That is, in addition to adapting the definitions of the constructs of our frameworks, we should also have an explicit conversation about what is context and how context defines the boundaries of these definitions, our evidence, and who judges the usefulness of the frameworks and theories in which context.…”

“…This diversity can create challenges for collaboration between disciplines, as concepts, terms and principles can seem unfamiliar to the non-specialist. However, implementation science does not stem from an existing cultural historical framework; rather a uniquely pragmatic approach has been taken to promote interdisciplinary collaboration, with the common goal to change practice and enhance patient outcomes (Boulton et al, 2020). The emergence of implementation science into the mainstream health care agenda provides an opportunity to bridge discipline-specific approaches to improve health care quality, including those from health organisation and management.…”

“…Through the lens of a socio-ecological system (Figure 2), we can observe how different levels of the health system are interdependent; acknowledging that to address "wicked problems" such as care quality, patient safety and ensuring valuebased care, we require greater understanding of the social behaviours and complex processes involved in delivering health care, beyond the clinical effectiveness of interventions. Implementation science has begun to broaden the scope of evidence-based medicine to elucidate how this clinical practice change occurs, incorporating perspectives from diverse scientific disciplines and methodologies (Boulton et al, 2020;Brown et al, 2017).…”
